【摘要】：目的:觀察二陳湯加味對(duì)慢性阻塞性肺疾病急性加重期(acute exacerbation period of COPD,AECOPD)患者肺功能、細(xì)胞和體液免疫功能,細(xì)胞趨化因子18(chemoattractant cytokine18,CCL18),白細(xì)胞介素-8(interleukin-8,IL-8),Clara細(xì)胞蛋白(Clara cell protein,CC16)和可溶性細(xì)胞間黏附分子-1(soluble intercellular adhesion molecular-1,s ICAM-1)的影響。方法:選取符合納入標(biāo)準(zhǔn)的老年AECOPD患者120例,分為治療組與對(duì)照組,每組60例。兩組均在常規(guī)治療的基礎(chǔ)上,治療組給予二陳湯加味,每日2次,療程14 d。14 d后評(píng)價(jià)兩組患者臨床療效,檢測(cè)肺功能、免疫球蛋白、外周血淋巴細(xì)胞亞群,酶聯(lián)免疫吸附試驗(yàn)(ELISA)測(cè)定患者血漿和呼氣冷凝液(exhale breath condensat,EBC)中CCL18,CC16,IL-8,s ICAM-1濃度。結(jié)果:與對(duì)照組治療后比較,治療組總有效率高于對(duì)照組(P0.05);治療組1 s用力呼氣容積(FEV_1),1 s用力呼氣量/用力肺活量(FVC)均增大(P0.01);炎細(xì)胞總數(shù)、中性粒細(xì)胞數(shù)、單核細(xì)胞和淋巴細(xì)胞均減少(P0.05)。與本組治療前比較,Ig G,Ig A,Ig M無明顯差異;CD3~+,CD4~+,CD4~+/CD8~+均增加,CD8~+降低(P0.05);血漿CCL18,CC16,IL-8,s ICAM-1均降低(P0.05),EBC中CCL18,CC16均降低(P0.05)。結(jié)論:二陳湯加味可調(diào)節(jié)T淋巴細(xì)胞亞群功能,增強(qiáng)細(xì)胞免疫,降低老年AECOPD患者血漿,EBC中CCL18,CC16,IL-8,s ICAM-1水平,對(duì)老年AECOPD有一定的抗炎作用。
[Abstract]:Objective: to observe the effects of Erchen decoction on pulmonary function, cellular and humoral immunity in patients with acute exacerbation period of chronic obstructive pulmonary disease (COPD). The effects of chemokine 18(chemoattractant on CCL18P, interleukin-8 (IL-8), Clara cell protein (CC16) and soluble intercellular adhesion molecule-1 intercellular adhesion (ICAM-1) were observed. Methods: 120 elderly patients with AECOPD were divided into treatment group and control group with 60 cases in each group. On the basis of routine treatment, the treatment group was treated with Erchen decoction twice a day. After 14 days of treatment, the clinical efficacy was evaluated, pulmonary function, immunoglobulin and peripheral blood lymphocyte subsets were detected. Enzyme linked immunosorbent assay (Elisa) was used to determine the concentration of CCL18 CC16 / IL-8 ICAM-1 in plasma and exhale breath condensate (EBC). Results: compared with the control group, the total effective rate of the treatment group was higher than that of the control group (P 0.05), the forced expiratory volume of 1 s and FEV / FVCC of the treatment group were all increased, the total number of inflammatory cells, the number of neutrophils, the number of neutrophilic granulocytes in the treatment group were higher than those in the control group. Both monocytes and lymphocytes decreased P0.05. There was no significant difference in plasma CCL18CC16, IL-8s ICAM-1 and the levels of CCL18CC16 / IL-8s ICAM-1 in P0.05, and the levels of CCL18CC16 and CCL18CC16 in EBC were lower than those before treatment. Conclusion: Erchen decoction can regulate the function of T lymphocyte subsets, enhance cellular immunity, decrease the level of CCL18, CC16 and IL-8s ICAM-1 in plasma of elderly AECOPD patients, and have certain anti-inflammatory effect on senile AECOPD.
