【摘要】：目的探討抗感染序貫纖維支氣管鏡高頻電刀介入治療及化療對(duì)肺癌合并阻塞性肺炎患者臨床療效,為臨床診治提供參考依據(jù)。方法選取2010年1月-2014年12月醫(yī)院收治的80例肺癌并發(fā)阻塞性肺炎患者作為研究對(duì)象,收集患者痰液標(biāo)本檢測(cè)病原菌,并行藥敏試驗(yàn)觀察耐藥性,及治療前后臨床指標(biāo)比較,數(shù)據(jù)采用SPSS 18.0軟件進(jìn)行統(tǒng)計(jì)分析。結(jié)果 80例患者送檢92份標(biāo)本,共檢出116株病原菌,以革蘭陰性菌為主,共67株占56.78%;肺炎克雷伯菌、銅綠假單胞菌對(duì)美羅培南、亞胺培南敏感性較低,均15.00%;第1秒呼出氣體體積(FEV1)、最大肺活量(FVC)及卡氏評(píng)分(KPS)評(píng)分、氣道腔徑與治療前比較均顯著升高,治療前后比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論肺癌合并阻塞性肺炎患者采用抗感染序貫纖維支氣管鏡高頻電刀介入治療、化療臨床療效較好,可解除阻塞性肺炎臨床癥狀,提高患者生存質(zhì)量。
[Abstract]:Objective to investigate the clinical efficacy of high frequency electric knife intervention and chemotherapy in patients with lung cancer complicated with obstructive pneumonia by sequential fiberoptic bronchoscopy (fiberoptic bronchoscope), and to provide reference for clinical diagnosis and treatment. Methods from January 2010 to December 2014, 80 patients with lung cancer complicated with obstructive pneumonia were selected as study objects. Sputum samples were collected to detect pathogenic bacteria, drug resistance was observed by drug sensitivity test, and clinical indexes were compared before and after treatment. The data were analyzed by SPSS 18.0 software. Results A total of 116 pathogenic bacteria were detected in 92 specimens from 80 patients, of which 67 were Gram-negative bacteria, which accounted for 56.78.The susceptibility of Klebsiella pneumoniae and Pseudomonas aeruginosa to meropenem and imipenem was low. The volume of gas exhaled in the first second, the maximal vital capacity (FVC), the KPS score and the airway cavity diameter were significantly higher than those before treatment, and the difference before and after treatment was statistically significant (P 0.05). Conclusion the patients with lung cancer complicated with obstructive pneumonia were treated with high frequency electric knife with antiinfective sequential fiberoptic bronchoscope. The clinical effect of chemotherapy is good, which can relieve the clinical symptoms of obstructive pneumonia and improve the quality of life of the patients.
