阻塞性睡眠呼吸暫停低通氣綜合征合并高血壓患者早晨血壓高峰出現(xiàn)率及出現(xiàn)時間研究

【摘要】:目的探討阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)合并高血壓患者動態(tài)血壓特點(diǎn),尤其是早晨血壓高峰出現(xiàn)率及出現(xiàn)時間,并分析其相關(guān)因素。方法選取2011年1月—2012年12月因打鼾在廣西壯族自治區(qū)人民醫(yī)院睡眠呼吸疾病診療中心行多導(dǎo)睡眠圖(PSG)檢測確診為OSAHS患者,根據(jù)有無高血壓分為OSAHS高血壓組和單純OSAHS組。同時,在高血壓門診隨機(jī)選取無打鼾的高血壓患者為對照1組(單純高血壓組),在體檢中心和我院職工中隨機(jī)招募無打鼾無高血壓者為對照2組(正常組)。對4組人群行24 h動態(tài)血壓監(jiān)測并對其數(shù)據(jù)進(jìn)行分析,比較4組非杓型血壓發(fā)生率、早晨血壓高峰出現(xiàn)率和出現(xiàn)的時間。結(jié)果 OSAHS高血壓組非杓型血壓發(fā)生率為86.4%(38/44),單純OSAHS組為79.1%(91/115),單純高血壓組為45.8%(11/24),正常組為23.3%(7/30),4組比較差異有統(tǒng)計(jì)學(xué)意義(P0.01);OSAHS高血壓組早晨血壓高峰發(fā)生率為84.1%(37/44),單純OSAHS組為50.4%(58/115),單純高血壓組為37.5%(11/24),正常組為26.7%(8/22),4組比較差異有統(tǒng)計(jì)學(xué)意義(P0.01)。早晨血壓高峰出現(xiàn)時間:OSAHS高血壓組5:00~6:00出現(xiàn)率最高,為86.4%(38/44);單純OSAHS組5:00~6:00出現(xiàn)率最高,為60.9%(70/115);單純高血壓組7:00~8:00出現(xiàn)率最高,為37.5%(5/24);正常組在早晨各時段出現(xiàn)率無差異。結(jié)論 OSAHS高血壓組與單純OSAHS組、單純高血壓組、正常組全天血壓變化規(guī)律存在差異,OSAHS高血壓組非杓型血壓及早晨血壓高峰發(fā)生率明顯增加,OSAHS高血壓組、單純OSAHS組早晨血壓高峰出現(xiàn)時間比單純高血壓組出現(xiàn)時間提前。
[Abstract]:Objective to investigate the characteristics of ambulatory blood pressure (ABP) in patients with obstructive sleep apnea hypopnea syndrome (OSAHS) complicated with hypertension, especially the occurrence rate and time of morning peak blood pressure, and to analyze the related factors.Methods from January 2011 to December 2012, patients with OSAHS were diagnosed by polysomnography (PSG) in the diagnosis and treatment center of sleep respiratory diseases of Guangxi Zhuang Autonomous region people's Hospital for snoring. According to hypertension, they were divided into OSAHS hypertension group and simple OSAHS group.At the same time, the hypertension patients without snoring were randomly selected as control group (simple hypertension group), and those without snoring and hypertension were recruited randomly in physical examination center and staff of our hospital as control group (normal group).The 24 h ambulatory blood pressure monitoring was performed in 4 groups and the data were analyzed to compare the incidence of non dipper blood pressure in the four groups and the occurrence rate and time of peak blood pressure in the morning.Results the incidence of non-dipper blood pressure in the OSAHS hypertension group was 86.4 / 44, that in the simple OSAHS group was 79.1 / 91 / 115, that in the simple hypertension group was 45.8 / 24, and that in the normal group was 23.3 / 7 / 30 / 4. There was a statistically significant difference in the incidence of morning blood pressure peak in the hypertension group (84.1% 3744%), in the simple OSAHS group (50.458% / 115).In the hypertension group, there was a significant difference (P 0.01) between 37.5 / 24% and 26.7 / 22 / 24 of the normal group (P < 0.01).Morning blood pressure peak time: 5: 00: 6: 00 the highest rate was 86.4B / 44%; the OSAHS group had the highest rate of 5: 00 / 6: 00 (60.970 / 115); the simple hypertension group had the highest occurrence rate of 7: 00 / 800 (37.55524%); the normal group had no difference in the rate of occurrence at each time in the morning.Conclusion there is a significant difference between OSAHS hypertension group and simple OSAHS group, simple hypertension group, normal group. The incidence of non-dipper blood pressure and morning blood pressure peak in OSAHS hypertension group is significantly higher than that in OSAHS hypertension group.The onset time of peak blood pressure in OSAHS group was earlier than that in simple hypertension group.
