【摘要】：目的：觀察應(yīng)用理氣活血法治療自發(fā)性氣胸（氣滯血瘀型）的臨床療效并探討其作用機制，判定理氣活血藥物治療本病證的有效性、安全性。 方法：將符合本研究納入標準的60例患者按照隨機、對照的原則分為試驗組32例，對照組28例。試驗組在西醫(yī)治療基礎(chǔ)上加用理氣活血中藥方劑，即血府逐瘀湯加青皮、佛手，水煎300ml，早晚兩次餐后半小時溫服，每日1劑；對照組采取單純西醫(yī)治療方法。觀察兩組患者綜合療效、留置引流管時間、住院時間及復(fù)發(fā)率，并對各項結(jié)果進行統(tǒng)計學(xué)分析。 結(jié)果：在綜合療效、留置引流管時間、住院時間及復(fù)發(fā)率方面，試驗組均優(yōu)于對照組，并具有統(tǒng)計學(xué)意義。 結(jié)論：理氣活血藥應(yīng)用于自發(fā)性氣胸（氣滯血瘀型），可以加快胸膜腔內(nèi)氣體吸收，促進胸膜破裂口愈合，與西醫(yī)常規(guī)治療方法聯(lián)合應(yīng)用，可以提高療效，縮短住院時間，降低復(fù)發(fā)率。其機理可能與理氣活血藥物能夠改善微循環(huán)及供氧，抗炎，緩解氣道阻塞，，防止氣體陷閉，促進受損部位增生、修復(fù)，保護血管內(nèi)皮細胞，調(diào)節(jié)免疫力等作用有關(guān)。
[Abstract]:Objective: to observe the clinical effect of regulating qi and activating blood circulation in treating spontaneous pneumothorax (qi stagnation and blood stasis type) and explore its mechanism.Methods: 60 patients who met the criteria were randomly divided into two groups: the experimental group (n = 32) and the control group (n = 28).Experimental group on the basis of western medicine treatment plus Liqi Huoxue prescription, namely Xuefu Zhuyu decoction Qingpi, Buddha's hand, decoction 300ml, in the morning and evening half an hour after the meal, 1 dose per day; the control group was treated with simple western medicine.The comprehensive curative effect, the time of indwelling drainage tube, the time of hospitalization and the recurrence rate of the two groups were observed, and the results were analyzed statistically.Results: the experimental group was superior to the control group in the comprehensive curative effect, the time of indwelling drainage tube, the time of hospitalization and the recurrence rate.Conclusion: the application of qi regulating and promoting blood circulation medicine in spontaneous pneumothorax (Qi stagnation and blood stasis type) can accelerate the gas absorption in pleural cavity, promote the healing of ruptured pleural cavity, and combine with the routine treatment of western medicine, which can improve the curative effect and shorten the hospital stay.Reduce the recurrence rate.The mechanism may be related to the effects of regulating qi and activating blood circulation on improving microcirculation and supplying oxygen, anti-inflammation, relieving airway obstruction, preventing gas trap, promoting the proliferation of damaged parts, repairing, protecting vascular endothelial cells, regulating immunity and so on.
