肺結(jié)核患者氧化應(yīng)激水平和膳食攝入情況分析

【摘要】:目的本文通過(guò)調(diào)查了解某地區(qū)肺結(jié)核患者機(jī)體氧化應(yīng)激水平及維生素A、E和β-胡蘿卜素營(yíng)養(yǎng)狀況,同時(shí)調(diào)查肺結(jié)核患者日常膳食攝入狀況,為指導(dǎo)患者合理膳食,改善肺結(jié)核患者營(yíng)養(yǎng)不良、提高機(jī)體抗病能力和治療效果提供科學(xué)依據(jù)。 方法隨機(jī)抽取臨沂市某縣肺結(jié)核防治所2010年就診的70例肺結(jié)核患者作為病例組,匹配選取該地同期正常健康人群70例作為對(duì)照組。兩組人群均進(jìn)行相關(guān)營(yíng)養(yǎng)指標(biāo)檢測(cè),包括基本營(yíng)養(yǎng)狀況,如體質(zhì)指數(shù)(BMI)、血紅蛋白(Hb);血脂水平分析,如甘油三酯(TG)、總膽固醇(TCHO)、高密度脂蛋白(HDL);抗氧化酶水平分析,包括超氧化物歧化酶(SOD)和過(guò)氧化氫酶(CAT);機(jī)體維生素A(VA)、維生素E(VE)和β-胡蘿卜素(β-C)水平。兩組人群應(yīng)用食物頻率表收集調(diào)查對(duì)象消費(fèi)的食物種類,做出相應(yīng)的膳食多樣化評(píng)分(DDS),分析并評(píng)價(jià)其日常膳食攝入狀況。 結(jié)果病例組研究對(duì)象的BMI為19.13kg/m2,明顯低于對(duì)照組人群21.95kg/m2(p0.05),而病例組Hb為128.36g/L,對(duì)照組為131.30g/L,但未觀察到顯著性差異(p0.05);血脂水平分析顯示病例組TG、TCHO和HDL含量分別為1.54mmol/L、4.47mmol/L和1.21mmol/L,均低于對(duì)照組相應(yīng)的含量(1.63mmol/L、5.20mmol/L、1.30mmol/L)(p值均0.05);抗氧化酶水平比較顯示病例組的SOD和CAT水平分別為78.20U/ml和5.24U/m1,對(duì)照組水平分別為83.27U/m1和9.99U/ml,病例組明顯低于對(duì)照組(p0.05);維生素水平分析顯示病例組的VA和VE含量0.256μg/ml和0.385μg/ml,均顯著低于對(duì)照組的0.385μg/ml和1.182μg/ml (p0.05),但病例組人群的β-C含量為0.048μg/ml,略低于對(duì)照組的0.051μg/ml (p0.05)。膳食多樣化評(píng)分(DDS)結(jié)果顯示,病例組人群DDS評(píng)分為4.81,對(duì)照組人群DDS評(píng)分為5.21。DDS評(píng)分比例病例組以4分最高,占病例組人群的25.7%;對(duì)照組以5分最高,占對(duì)照組人群的58.6%。DDS評(píng)分能夠達(dá)到7分及7分以上的比例占病例組和對(duì)照組分別為15.7%和17.1%。膳食攝入分布情況分析顯示病例組人群攝入谷類、肉類、蔬菜類、水果類和豆類食物顯著低于對(duì)照組人群(p0.05)。 結(jié)論肺結(jié)核患者的氧化應(yīng)激水平升高,患者體內(nèi)抗氧化酶和VA、VE、β-C水平均顯著低于正常人群,而且膳食攝入不均衡,谷類、肉類、蔬菜類、水果類和豆類食物攝入較正常人群少。建議積極指導(dǎo)患者改善膳食結(jié)構(gòu),加強(qiáng)對(duì)維生素的補(bǔ)充,防止各種營(yíng)養(yǎng)素缺乏。
[Abstract]:Objective to investigate the level of oxidative stress and vitamin A E and 尾 -carotene nutrition in pulmonary tuberculosis patients in a certain area, and to investigate the status of daily dietary intake of pulmonary tuberculosis patients in order to guide the reasonable diet of patients. To improve the malnutrition of pulmonary tuberculosis patients, improve the body disease resistance and therapeutic effect to provide scientific basis. Methods A total of 70 pulmonary tuberculosis patients in 2010 were randomly selected as the case group, and 70 healthy people in the same period were matched as the control group. The related nutritional indexes were detected in both groups. These include basic nutritional status, such as body mass index (BMI), hemoglobin (HB), lipid level analysis, such as triglyceride (TGN), total cholesterol (TCHO), high density lipoprotein (HDL), antioxidant enzymes, These include superoxide dismutase (SOD) and catalase (catalase), vitamin A, vitamin E (VEV) and 尾 -carotene (尾 -C.) levels. To analyze and evaluate the status of daily dietary intake by making the corresponding dietary diversity score (DDS). Results the BMI of the case group was 19.13 kg / m ~ (2), which was significantly lower than that of the control group (21.95 kg / m ~ (2) p _ (0.05)), while the HB of the case group was 128.36 g / L and the control group was 131.30 g / L, but there was no significant difference between the two groups (p 0.05). The blood lipid level analysis showed that the levels of TGTCHO and HDL were 1.54 mmol / L 4.47 mmol / L and 1.21 mmol / L, respectively. Compared with the control group, the levels of SOD and CAT were 78.20U/ml and 5.24U / ml, respectively, and those of the control group were 83.27U/m1 and 9.99U / ml, respectively, which were significantly lower than those of the control group (p0.05), vitamin level analysis showed that the level of antioxidant enzyme was lower than that of the control group (P < 0.05). The level of antioxidant enzyme in the case group was significantly lower than that in the control group (P < 0.05), and the levels of SOD and CAT were 5.24U / ml and 9.99U / ml, respectively, which were significantly lower than those in the control group. VA and VE contents of 0.256 渭 g/ml and 0.385 渭 g / ml in the case group were significantly lower than those in the control group (0.385 渭 g/ml and 1.182 渭 g/ml / ml), but the 尾 -C content in the case group was 0.048 渭 g / ml, which was slightly lower than that in the control group (0.051 渭 g/ml / ml, p 0.05). The DDS score of the case group was 4.81, the DDS score of the control group was 5.21.DDS score, the proportion of the case group was the highest, accounting for 25.7% of the case group, and the control group had the highest score of 5 points. The 58.6%.DDS scores of the control group were 15.7% and 17.1% respectively. The distribution of dietary intake showed that the population of the case group ate cereals, meat and vegetables. Fruits and legumes were significantly lower than that of control group (P 0.05). Conclusion the level of oxidative stress in patients with pulmonary tuberculosis is increased, the levels of antioxidant enzymes, VAtoVEand 尾 -C in patients with pulmonary tuberculosis are significantly lower than those in the normal population, and the dietary intake is not balanced, cereals, meat, vegetables, etc. The food intake of fruits and legumes is less than that of normal people. It is suggested that the patients should be actively instructed to improve their dietary structure, to strengthen the supplement of vitamins and to prevent the deficiency of all kinds of nutrients.
