高頻振蕩通氣對(duì)急性呼吸窘迫綜合征幼豬氧合和肺傷標(biāo)志物的影響

【摘要】:目的探討相比于小潮氣量肺保護(hù)性常規(guī)機(jī)械通氣(conventional mechanicalventilation, CMV),高頻振蕩通氣(high frequency oscillatory ventilation, HFOV)對(duì)生理鹽水灌洗所致急性呼吸窘迫綜合征(acute respiratory distress syndrome, ARDS)幼豬低氧血癥的治療效果并通過(guò)測(cè)定血清Clara細(xì)胞分泌蛋白(clara cell secretion protein,CC16)、可溶性細(xì)胞間粘附分子-1(soluble intercellular adhesion molecule-1,sICAM-1)和高遷移率蛋白-1(high mobility groupbox-1,HMGB-l)三個(gè)肺損傷生物標(biāo)志物水平評(píng)估HFOV對(duì)肺損傷的影響。 方法12只幼豬在氣管插管全身麻醉下經(jīng)右側(cè)頸內(nèi)靜脈安置中心靜脈導(dǎo)管,經(jīng)右側(cè)股動(dòng)脈安置PICCO熱稀釋導(dǎo)管,并行膀胱造瘺安置導(dǎo)尿管,用38℃生理鹽水經(jīng)氣管導(dǎo)管反復(fù)灌洗制造ARDS模型,隨機(jī)分為常規(guī)機(jī)械通氣組(CMV組)和高頻振蕩通氣組(HFOV組),記錄兩組造模前(T0)、造模成功后(T1)、機(jī)械通氣治療后0.5小時(shí)(T2)、1小時(shí)(T3)、2小時(shí)(T4)、4小時(shí)(T5)的氧合指標(biāo),測(cè)量T0、T1、T5點(diǎn)血清CC16、sICAM-1和HMGB-1水平。 結(jié)果造模前后幼豬PaO2、氧合指數(shù)、DO2、下降,PaCO2、Qs/Qt和VO2升高,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。造模后,CMV和HFOV兩組隨機(jī)械通氣治療時(shí)間延長(zhǎng)PaO2和氧合指數(shù)自T2時(shí)間點(diǎn)進(jìn)行性升高,組內(nèi)差異均有統(tǒng)計(jì)學(xué)意義(P0.05),HFOV組升高幅度更為明顯,自T3時(shí)間點(diǎn)有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組均有DO2升高(P0.05),但組間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。兩組Qs/Qt均有減少的趨勢(shì),,在T5點(diǎn)有統(tǒng)計(jì)學(xué)意義(P0.05),但組間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。兩組VO2和PaCO2通氣治療前后組間組內(nèi)比較均無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。兩組血清CC16和sICAM-1在造模成功后T1點(diǎn)較T0點(diǎn)均有明顯升高(P0.05),并隨通氣治療CC16和sICAM-1均有輕微下降趨勢(shì),但組間組內(nèi)比較無(wú)差異(P0.05)。兩組HMGB-1自造模后進(jìn)行性升高,T5點(diǎn)較T1點(diǎn)具有統(tǒng)計(jì)學(xué)差異(P0.05),但組間差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。 結(jié)論相比于小潮氣量肺保護(hù)性CMV,HFOV更為快速有效的改善了ARDS的低氧血癥,并且沒(méi)有明顯的二氧化碳潴留和氧耗增加。在呼吸機(jī)相關(guān)性肺損傷方面,通過(guò)三個(gè)肺損傷生物標(biāo)志物的觀察,HFOV沒(méi)有加重肺損傷和炎癥反應(yīng)程度,但也沒(méi)有發(fā)揮其理想的肺保護(hù)作用。
[Abstract]:Objective to investigate the therapeutic effect of conventional mechanical ventilation (CMV) and high frequency oscillatory ventilation (HFOV) on acute respiratory distress syndrome (ARDS) induced by saline lavage in young pigs with acute respiratory distress syndrome (ARDS). The effects of HFOV on lung injury were evaluated by measuring the levels of serum Clara cell secretory protein clara cell secretion protein CC16, soluble intercellular adhesion molecule-1 (ICAM-1) and high mobility high mobility group box-1 (HMGB-l). Methods under general anesthesia, 12 young pigs were placed with central catheter via right internal jugular vein, PICCO heat dilute catheter via right femoral artery, and catheterization with cystostomy.
