【摘要】：目的：探討慢性阻塞性肺疾病（COPD）患者急性加重期治療前后血清D-二聚體（D-D）、基質金屬蛋白酶-9（MMP-9）及基質金屬蛋白酶抑制劑-1（TIMP-1）水平的變化及意義。 方法：選取COPD急性加重期患者37例，正常對照組30例，用酶聯(lián)免疫吸附（ELISA）方法檢測其治療前后血清中D-二聚體、MMP-9及TIMP-1的水平。結果：（1）COPD組治療前血清D-二聚體、MMP-9及TIMP-1的水平均高于正常對照組；差別均具有統(tǒng)計學意義（P＜0.01）；COPD組治療后D-二聚體、MMP-9及TIMP-1的水平也均高于正常對照組，差別均具有統(tǒng)計學意義（P＜0.05），尤其是TIMP-1差異更為顯著（P㩳0.01）；COPD組治療后D-二聚體、MMP-9及TIMP-1的水平明顯低于治療前，差異均有統(tǒng)計學意義（P㩳0.05）。（2）COPD組治療前后，D-二聚體與MMP-9、D-二聚體與TIMP-1及MMP-9與TIMP-1均具有良好的正相關性，，且治療前的相關系數(shù)大于治療后。 結論：（1）COPD急性加重期患者血清D-二聚體、MMP-9及TIMP-1的濃度水平增高，體內可能存在高凝狀態(tài)或血栓前狀態(tài)，治療后有一定程度改善；（2）COPD患者中D-二聚體與MMP-9有良好的正相關性，二者可作為COPD患者的疾病急時相反應因子；TIMP-1與MMP-9在治療前后水平變化的一致程度有一定差異，影響二者表達水平的因素復雜多樣，在COPD急性加重期患者中二者的失衡表現(xiàn)尤為突出。
[Abstract]:Objective: to investigate the changes and significance of serum D dimer D-DG, matrix metalloproteinase-9 (MMP-9) and matrix metalloproteinase inhibitor (MMP-1) in patients with chronic obstructive pulmonary disease (COPD) during acute exacerbation. Methods: 37 patients with acute exacerbation of COPD and 30 normal controls were selected. The serum levels of MMP-9 and TIMP-1 were detected by Elisa before and after treatment. Results the serum levels of MMP-9 and TIMP-1 in TIMP-1 group were higher than those in normal control group before and after treatment. The difference was statistically significant (P < 0.01). The levels of MMP-9 and TIMP-1 in the TIMP-1 group were higher than those in the normal control group (P < 0.05), especially in the TIMP-1 group. After treatment, the levels of MMP-9 and TIMP-1 were significantly lower in the 0. 01 TIMP-1 group than before treatment, and the difference was statistically significant (P < 0. 01). There were good positive correlations between D-dimer and MMP-9 D- dimer and TIMP-1, MMP-9 and TIMP-1 before and after treatment, and the correlation coefficient before and after treatment was higher than that before and after treatment. Conclusion the serum levels of MMP-9 and TIMP-1 in patients with acute exacerbation of TIMP-1 may be hypercoagulable or prethrombotic, and there is a good positive correlation between D- dimer and MMP-9 after treatment. The level of TIMP-1 and MMP-9 in patients with COPD before and after treatment were different, and the factors affecting their expression were complex and varied. In patients with acute exacerbation of COPD, the imbalance between the two is particularly prominent.
