【摘要】：目的研究慢性阻塞性肺疾病急性加重期患者肺部真菌感染對(duì)患者血清炎癥因子和腫瘤壞死因子的影響。方法選取2014年6月-2016年4月慢性阻塞性肺疾病急性加重期合并真菌感染的患者48例為AECOPD感染組,另外50例慢性阻塞性肺疾病且無真菌感染的COPD患者為未感染組,同時(shí)取50例健康志愿者為對(duì)照組,檢測(cè)感染組與未感染組患者的肺功能并進(jìn)行分級(jí),以及感染組患者感染的病原菌菌種。檢測(cè)各組患者血清炎癥細(xì)胞因子(IL-1β、IL-2、IL-4、IL-6、IL-8、IL-10)、腫瘤壞死因子(TNF-α)、干擾素(IFN-γ)的表達(dá)量,對(duì)比三組患者血清炎癥細(xì)胞因子及不同肺功能分級(jí)間血清炎癥因子之間的差異。結(jié)果與對(duì)照組相比,感染組與未感染組患者血清炎癥因子(IL-2、IL-6)及腫瘤壞死因子TNF-α含量顯著升高,其中感染組IL-2、IL-6及TNF-α含量最高,顯著高于未感染組,IFN-γ含量顯著降低(P0.05)。此外,未感染組患者不同肺功能分級(jí)間血清IL-1β、TNF-α存在顯著差異,以Ⅲ級(jí)IL-1β、TNF-α含量最高(P0.05);急性感染組患者不同肺功能分級(jí)間血清IL-2、IL-4及TNF-α差異有統(tǒng)計(jì)學(xué)意義,以Ⅲ級(jí)IL-2、IL-4含量最高(P0.05)。結(jié)論血清IL-2、IL-6、TNF-α含量測(cè)量對(duì)AECOPD合并真菌感染患者病情的評(píng)估及感染預(yù)測(cè)具有重要臨床意義。
[Abstract]:Objective to study the effect of pulmonary fungal infection on serum inflammatory factor and tumor necrosis factor in patients with acute exacerbation of chronic obstructive pulmonary disease. Methods from June 2014 to April 2016, the patients with acute exacerbation of chronic obstructive pulmonary disease were selected. 48 patients with fungal infection were infected with AECOPD. Another 50 COPD patients with chronic obstructive pulmonary disease without fungal infection were selected as control group and 50 healthy volunteers as control group. The pulmonary function of infected and uninfected patients was measured and graded. The levels of serum inflammatory cytokines (IL-1 尾, IL-2, IL-4, IL-6, IL-8, IL-10, TNF- 偽, IFN- 緯) in patients with infection were detected, and the expression levels of IL-10, TNF- 偽 and IFN- 緯 were measured. Results compared with the control group, the levels of serum inflammatory cytokines and tumor necrosis factor TNF- 偽 in the infected and uninfected patients were significantly higher than those in the control group. The levels of IL-6 and TNF- 偽 in the infected group were the highest, which were significantly higher than those in the uninfected group. In addition, there were significant differences in serum IL-1 尾 -TNF- 偽 between different lung function grades in the uninfected group. The level of TNF- 偽 was the highest in grade 鈪，
