【摘要】：目的:評(píng)估肺結(jié)節(jié)熱消融治療后胸膜損傷的CT表現(xiàn),探討其與術(shù)后并發(fā)癥的關(guān)系。方法:對(duì)我院2013年7月-2016年7月40例患者的57個(gè)肺結(jié)節(jié)進(jìn)行CT引導(dǎo)下熱消融治療,回顧性分析術(shù)后消融區(qū)域的CT改變和并發(fā)癥發(fā)生率。結(jié)果:40例患者均完成熱消融治療,當(dāng)消融區(qū)域鄰近胸膜時(shí),臟層胸膜出現(xiàn)局部凹陷,本組研究中8例患者出現(xiàn)該征象,其中6例(75%)術(shù)后發(fā)生胸膜相關(guān)并發(fā)癥,4例(50%)行胸腔閉式引流。結(jié)論:肺結(jié)節(jié)熱消融術(shù)后局部臟層胸膜凹陷可能是胸膜損傷的表現(xiàn),提示患者有較高的胸膜相關(guān)并發(fā)癥風(fēng)險(xiǎn)。
[Abstract]:Objective: to evaluate the CT findings of pleural injury after thermal ablation of pulmonary nodules, and to explore the relationship between CT findings and postoperative complications. Methods: 57 pulmonary nodules in 40 patients from July 2013 to July 2016 in our hospital were treated with CT guided thermal ablation. Results 40 patients were treated with thermal ablation. When the ablation area was adjacent to the pleura, there was a local depression in the visceral pleura. In this study, 8 patients had this sign. Pleural associated complications occurred in 6 cases (50%) and closed pleural drainage was performed in 4 cases. Conclusion: the pleural depression in the local layer after thermal ablation of pulmonary nodules may be the manifestation of pleural injury, suggesting that the patients have a high risk of pleural complications.
