【摘要】：目的探討急診ICU老年患者肺部感染的臨床特點(diǎn)及相關(guān)危險(xiǎn)因素,并針對(duì)高危因素分析其預(yù)防對(duì)策。方法選取2013年2月-2016年2月醫(yī)院急診ICU收治的老年患者866例,研究老年患者肺部感染的臨床特征及感染病原菌分布,采用單因素與多因素分析導(dǎo)致ICU老年患者肺部感染的高危因素,并進(jìn)行統(tǒng)計(jì)學(xué)檢驗(yàn)。結(jié)果肺部感染患者的主要臨床表現(xiàn)是咳嗽咳痰、胸悶氣促,其中98.88%的患者X線檢查發(fā)現(xiàn)陽(yáng)性征象,痰培養(yǎng)結(jié)果顯示,肺炎克雷伯菌和銅綠假單胞菌構(gòu)成比相對(duì)較高,分別占39.32%和30.34%;單因素分析結(jié)果顯示,年齡、基礎(chǔ)疾病、侵入性操作與老年患者肺部感染的發(fā)生具有相關(guān)性(P0.05);多因素logistic回歸分析結(jié)果顯示,年齡≥80歲、基礎(chǔ)疾病≥2種、有侵入性操作是導(dǎo)致肺部感染的獨(dú)立危險(xiǎn)因素;89例肺部感染患者經(jīng)過(guò)治療,其中48例(53.93%)患者癥狀完全消失,17例(19.10%)患者癥狀緩解,18例(20.22%)患者病情惡化,6例(6.74%)患者死亡。結(jié)論 ICU老年患者肺部感染的高危因素包括年齡≥80歲、有≥2種基礎(chǔ)疾病以及侵入性操作,針對(duì)以上高危因素制定預(yù)防措施對(duì)于減少肺部感染具有積極作用。
[Abstract]:Objective to investigate the clinical characteristics and related risk factors of pulmonary infection in elderly patients with emergency ICU, and to analyze the preventive measures against the risk factors. Methods 866 elderly patients with emergency ICU were selected from February 2013 to February 2016 in our hospital. To study the clinical characteristics and pathogenic bacteria distribution of pulmonary infection in elderly patients, univariate and multivariate analysis was used to analyze the high risk factors of pulmonary infection in elderly patients with ICU. Results the main clinical manifestations of patients with pulmonary infection were cough and sputum, chest tightness and shortness of breath. Among them, 98.88% of the patients found positive signs by X-ray examination, and the results of sputum culture showed that, The composition ratios of Klebsiella pneumoniae and Pseudomonas aeruginosa were relatively high, accounting for 39.32% and 30.34 respectively.
