多種原發(fā)灶來源的胰腺轉(zhuǎn)移性腫瘤的超聲造影表現(xiàn)特征分析

目的:通過分析多種原發(fā)灶來源的胰腺轉(zhuǎn)移性腫瘤(pancreatic metastatic tumors,PM)的超聲造影(contrastenhancedultrasound,CEUS)表現(xiàn),旨在提高對PM的診斷效能。方法:回顧并分析自2015年1月—2019年9月于上海交通大學(xué)附屬第一人民醫(yī)院就診的13例PM患者的臨床及超聲資料,患者均在手術(shù)切除或穿刺介入前行常規(guī)超聲和CEUS檢查。結(jié)果:經(jīng)病理學(xué)檢查證實11例PM,原發(fā)灶來源為肺癌3例,胃腺癌3例,肝膽管細(xì)胞癌1例,甲狀腺濾泡癌1例,膽囊腺癌1例,膽總管腺癌1例,腎透明細(xì)胞癌1例;其余2例結(jié)合病史考慮為腹腔間葉源性腫瘤和肝細(xì)胞癌來源。常規(guī)超聲顯示,PM表現(xiàn)為邊界清晰的低回聲腫塊,回聲多均勻,僅4例彩色多普勒血流顯像探及血流信號。CEUS顯示,7例PM表現(xiàn)為向心性增強,6例為整體增強;在動脈期,7例呈高增強,6例呈低增強;在靜脈期,僅2例呈高增強,余11例呈低增強。結(jié)論:CEUS上PM可呈特異的向心性增強方式;根據(jù)原發(fā)灶來源不同,PM呈富血供或乏血供表現(xiàn)。
