ADC及rADC值對缺血性腦梗死缺血半暗帶的診斷價值

【摘要】:目的:探討表觀擴散系數(shù)(ADC)值及相對表觀擴散系數(shù)(rADC)值在評價缺血性腦梗死缺血半暗帶中的應用價值。方法:對42例超急性期腦梗死患者行常規(guī)CT、MRI、DSA檢查,并于24h后行CT及MRI復查,根據(jù)患者出現(xiàn)臨床癥狀到MRI檢查時間分為超急性期(發(fā)病6h以內(nèi))、急性期(6~24h)和亞急性早期(24h~7d)。對比分析不同時期患側(cè)梗死核心區(qū)、缺血半暗帶與健側(cè)鏡像區(qū)的ADC及rADC值。結(jié)果:超急性期、急性期及亞急性期梗死核心區(qū)ADC值及rADC值均低于健側(cè)鏡像區(qū),缺血半暗帶ADC值及rADC值僅輕度下降,平均降幅為20%。梗死核心區(qū)與缺血半暗帶ADC值及rADC值隨著發(fā)病時間延長,上升趨勢不同。缺血半暗帶ADC值及rADC值均高于梗死核心區(qū),差異有統(tǒng)計學意義(P0.01)。結(jié)論:ADC值及rADC值對判定缺血半暗帶具有較高的臨床應用價值,有望成為一種簡便易行的確定缺血半暗帶的檢查方法,為指導患者進行臨床治療提供重要的影像學依據(jù)。
