【摘要】：目的： 評(píng)價(jià)甲狀腺結(jié)節(jié)大小對(duì)細(xì)針穿刺細(xì)胞學(xué)(FNAC)診斷準(zhǔn)確性的影響。 方法： 分析我院2010年6月至2012年10月超聲診斷為甲狀腺結(jié)節(jié)的住院手術(shù)患者630例,共691個(gè)甲狀腺結(jié)節(jié),收集術(shù)前超聲引導(dǎo)下細(xì)針穿刺細(xì)胞學(xué)(FNAC)檢查的資料,依據(jù)Bethesda System細(xì)胞學(xué)分類方法,將細(xì)胞學(xué)結(jié)果分為Ⅵ級(jí)：良性、細(xì)胞不足或無(wú)法診斷、不典型增生、濾泡性腫瘤、可疑惡性及惡性6個(gè)級(jí)別,以大于Ⅳ級(jí)作為診斷甲狀腺惡性腫瘤標(biāo)準(zhǔn),結(jié)節(jié)大小按長(zhǎng)徑(L)分：A組(L0.5cm)、B組(0.5cmL1.0cm)、 C組(L1.Ocm),以術(shù)后病理結(jié)果為診斷金標(biāo)準(zhǔn)。分別計(jì)算出各組超聲引導(dǎo)下細(xì)針穿刺細(xì)胞學(xué)對(duì)甲狀腺結(jié)節(jié)鑒別診斷的敏感性、特異性、準(zhǔn)確率、陰性預(yù)測(cè)值、陽(yáng)性預(yù)測(cè)值、假陰性率及假陽(yáng)性率。 結(jié)果： 691個(gè)結(jié)節(jié)中A組176個(gè)(25.47%),B組298個(gè)(43.13%),C組217個(gè)(31.40%),三組間比較,B組超聲引導(dǎo)下FNAC診斷甲狀腺結(jié)節(jié)的準(zhǔn)確率(90.94%)明顯高于A組(80.11%)與C組(83.41%),差異具有統(tǒng)計(jì)學(xué)意義(P0.05),A組與C組間的差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)；而三組間特異性、陰性預(yù)測(cè)值、陽(yáng)性預(yù)測(cè)值的差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。 結(jié)論： 甲狀腺結(jié)節(jié)大小在一定程度上會(huì)影響超聲引導(dǎo)下細(xì)針穿刺細(xì)胞學(xué)診斷的準(zhǔn)確
[Abstract]:Purpose: Evaluation of the accuracy of thyroid nodule size in the diagnosis of fine needle aspiration cytology (FNAC) In response. Methods:630 patients with thyroid nodules from June 2010 to October 2012 were analyzed. A total of 691 thyroid nodules were collected. The cytologic results were divided into grade VI: benign, insufficient or undiagnosed, atypical hyperplasia, follicular tumor, suspicious malignant and malignant 6 levels, and the nodule size was divided by length-diameter (L): Group A (L0 (5 cm), group B (0.5 cmL1.0 cm) and group C (L1.
