【摘要】：目的探討腕管內(nèi)正中神經(jīng)的超聲解剖測(cè)量方法及各項(xiàng)指標(biāo),為臨床提供正常解剖參數(shù)。方法應(yīng)用高頻超聲檢查60側(cè)正常人腕管段正中神經(jīng),分別測(cè)量其腕管入口、中點(diǎn)和出口平面的橫截面積,腕管中點(diǎn)平面正中神經(jīng)的扁平率和屈肌支持帶的厚度。應(yīng)用超聲剪切波彈性成像技術(shù)測(cè)量30側(cè)腕管內(nèi)正中神經(jīng)近1/3段、中1/3段、遠(yuǎn)1/3段的硬度值。結(jié)果腕管入口、中點(diǎn)、出口平面正中神經(jīng)截面積分別為(0.09±0.02)、(0.08±0.02)、(0.10±0.03)cm2,不同部位截面積均值無(wú)統(tǒng)計(jì)學(xué)差異(P0.01);腕管中點(diǎn)平面正中神經(jīng)的扁平率3.12±0.29;屈肌支持帶厚度為(0.33±0.07)cm。正中神經(jīng)腕管近段、中段、遠(yuǎn)段最高硬度的分別為(41.3±10.3)、(35.4±5.8)、(38.1±4.3)k Pa,不同部位硬度均值無(wú)統(tǒng)計(jì)學(xué)差異(P0.05)。結(jié)論高頻超聲檢查為臨床提供了腕管內(nèi)正中神經(jīng)形態(tài)學(xué)解剖參數(shù),剪切波彈性成像提供了神經(jīng)硬度參數(shù)。兩者結(jié)合應(yīng)用為腕管綜合征的臨床診斷提供超聲解剖學(xué)依據(jù)。
[Abstract]:Objective to explore the ultrasonic anatomical measurement method and indexes of median nerve in carpal canal, and to provide normal anatomical parameters for clinic. Methods the median nerve of wrist canal segment was examined by high frequency ultrasound on 60 sides of normal subjects. The cross section area of the entrance, midpoint and outlet plane of carpal canal, the flattening rate of median nerve and the thickness of flexor supporting band were measured respectively. Ultrasonic shear wave elastic imaging technique was used to measure the hardness of the median nerve in 30 sides of the carpal canal from 1 to 3 segments, from 1 to 3 segments and from 1 to 3 segments. Results the cross section area of median nerve at the entrance, midpoint and outlet plane of carpal canal was (0.009 鹵0.002), () 鹵0.002), (0.10 鹵0.003) cm2, and there was no significant difference in the mean cross section area of different parts of carpal tunnel entrance, midpoint and outlet plane (P 0.01). The flattening rate of median nerve in the midpoint plane of carpal canal was 3.12 鹵0.29. the thickness of flexor supporting band was (0.33 鹵0.07) cm.. The highest hardness of the wrist canal in the proximal, middle and distal segments of the median nerve was (41. 3 鹵10. 3), (35.4 鹵5. 8), (38.1 鹵4. 3) k Pa,). There was no significant difference in the mean hardness of different parts of the median nerve carpal canal (P 0.05). Conclusion High frequency ultrasound provides morphological and anatomical parameters of median nerve in carpal canal and shear wave elastic imaging provides nerve hardness parameters. The combination of the two provides ultrasonic anatomical basis for the clinical diagnosis of carpal tunnel syndrome.
