【摘要】：目的應(yīng)用二維分層應(yīng)變技術(shù)評(píng)估蒽環(huán)類藥物化療后乳腺癌患者早期左心室收縮功能的變化。方法收集接受蒽環(huán)類藥物化療的乳腺癌患者32例及未接受化療的乳腺癌術(shù)后患者20例。化療患者于化療3個(gè)月、5個(gè)月后接受常規(guī)超聲心動(dòng)圖采集圖像,應(yīng)用EchoPAC軟件測(cè)量左心室長(zhǎng)軸各切面收縮期心內(nèi)膜下、中層和心外膜下心肌的整體縱向應(yīng)變(GLS)及各節(jié)段(基底段、中間段、心尖段)的縱向應(yīng)變。結(jié)果與對(duì)照組比較,隨著化療周期的增加,蒽環(huán)類藥物化療后乳腺癌患者心內(nèi)膜下、中層和心外膜下心肌GLS均減低(P均0.05),而左心室射血分?jǐn)?shù)(LVEF)無(wú)顯著差異(P0.05);與對(duì)照組比較,除各層心肌心尖段外,化療后患者各層心肌基底段、中間段縱向應(yīng)變減低(P均0.05)。結(jié)論縱向分層應(yīng)變技術(shù)可準(zhǔn)確評(píng)估乳腺癌蒽環(huán)類藥物化療后左心各層心肌整體及局部收縮功能,為判斷心肌損傷程度提供了新的方法。
[Abstract]:Objective to evaluate the changes of left ventricular systolic function in patients with breast cancer after anthracycline chemotherapy by two dimensional stratified strain technique. Methods 32 breast cancer patients receiving anthracycline chemotherapy and 20 breast cancer patients without chemotherapy were collected. After 3 months of chemotherapy and 5 months after chemotherapy, the patients received conventional echocardiography to collect images, and EchoPAC software was used to measure the subsystolic endocardium on the long axis of left ventricle. The overall longitudinal strain (GLS) of the middle and epicardial myocardium and the longitudinal strain of each segment (basal segment, middle segment, apical segment). Results compared with the control group, with the increase of chemotherapy cycle, the GLS of subendocardial, middle and epicardial myocardium in patients with breast cancer decreased after anthracycline chemotherapy (P0. 05). There was no significant difference in left ventricular ejection fraction (LVEF) (P0.05). Compared with the control group, the longitudinal strain in the basal and middle segments of the myocardium decreased after chemotherapy except the apical segment of the myocardium (all P 0.05). Conclusion Longitudinal stratification strain technique can accurately evaluate the global and local contractile function of left ventricular myocardium after anthracycline chemotherapy in breast cancer. It provides a new method for judging the degree of myocardial injury.
