超聲導管腎去交感化的有效性及安全性

【摘要】:目的探索自行研發(fā)10.0 MHz超聲導管以不同參數組合消融腎動脈周圍神經時溫度場的分布,并觀察在體條件下不同劑量聲能產生的去交感神經效果。方法超聲導管消融犬離體腎動脈,以針式熱電偶檢測不同聲功率消融不同時間時距腎動脈內膜不同距離處的溫度。選擇溫度≥51℃的組合行活體消融。測量消融前及消融后1周的血壓,取腎動脈及周圍組織行病理學檢查。結果腎動脈周圍溫度與消融功率及時間呈正相關,各組溫度均在5 mm處達峰值。1.5 W×30s消融組、3.0 W×15s消融組及3.0 W×30s消融組在5mm、7mm處達有效消融溫度。消融1周后消融組收縮壓和舒張壓下降幅度分別為(8.77±0.78)mmHg/(6.44±0.60)mmHg,(9.78±1.86)mmHg/(6.36±1.49)mmHg,(13.34±1.75)mmHg/(12.20±2.44)mmHg(P0.05)。肉眼觀測腎動脈周圍無損傷,HE染色見腎周圍神經空泡樣變。結論 10.0 MHz超聲導管消融腎周圍神經,在距內膜5~7mm處達有效溫度;在體實驗初步證明其應用于腎去交感的有效性及安全性。
