【摘要】：目的旨在探討非瓣膜性房顫血栓形成的心臟本身因素,二尖瓣反流與左心房血栓形成的關(guān)系。方法 212例心房纖顫患者接受經(jīng)胸超聲心動(dòng)圖和經(jīng)食道超聲檢查。按左心房有無血栓分為血栓組(50例)和無血栓組(162例)。分析入選病例的超聲心動(dòng)圖資料進(jìn)行分析,對(duì)單因素分析中有統(tǒng)計(jì)學(xué)意義的指標(biāo)行多因素回歸分析。結(jié)果發(fā)現(xiàn)血栓組和非血栓組左心房內(nèi)徑(LAD)[(48.1±4.7)mmvs.(40.4±3.4)mm]、左室舒張末徑(LVEDD)[(50.9±3.7)mmvs.(46.4±3.9)mm]、左室射血分?jǐn)?shù)(LVEF)[(56.5±3.9)%vs.(65.9±5.4)%],組間差異有統(tǒng)計(jì)學(xué)意義(P0.01),血栓組僅有少量反流,中度以上反流患者無血栓形成。結(jié)論 LAD、心功能下降是并發(fā)左心房血栓的危險(xiǎn)因素。二尖瓣反流是左心房血栓形成的保護(hù)性因素。
[Abstract]:Objective to investigate the relationship between mitral regurgitation and left atrial thrombosis in patients with non-valvular atrial fibrillation. Methods 212 patients with atrial fibrillation were examined by transthoracic echocardiography and transesophageal echocardiography. According to left atrial thrombus, 50 cases were divided into thrombus group (50 cases) and no thrombus group (162 cases). The echocardiographic data of the selected patients were analyzed and multivariate regression analysis was performed on the statistically significant indexes in univariate analysis. The results showed that left atrial diameter (LAD) [(48.1 鹵4.7) mmvs. (40.4 鹵3.4) mm], left ventricular end-diastolic diameter (LVEDD) [(50.9 鹵3.7) mmvs. (46.4 鹵3.9) mm] and left ventricular ejection fraction (LVEF) [(56.5 鹵3.9) vs. (65.9 鹵5.4)%] were significantly different between the two groups. Conclusion the decrease of LAD, cardiac function is the risk factor of left atrial thrombosis. Mitral regurgitation is a protective factor for left atrial thrombosis.
