【摘要】：目的:研究良惡性腋淋巴結(jié)的超聲造影特征,并評價超聲造影在乳腺癌患者術(shù)前腋淋巴結(jié)診斷中的價值。方法:回顧性分析65個良惡性腋淋巴結(jié)的超聲造影增強征象和時間-強度曲線定量參數(shù)(AT、TTP、PI),比較超聲造影和常規(guī)超聲對良惡性腋淋巴結(jié)的診斷率。結(jié)果:65個腋淋巴結(jié)造影增強模式表現(xiàn)為:Ⅰ型整體增強型,Ⅱ型周邊增強型,Ⅲ型局部增強型。造影增強模式在良惡性組中具有顯著性差異(P0.05),惡性多表現(xiàn)為整體增強型,良性多表現(xiàn)為周邊增強型。以造影增強Ⅰ型為惡性診斷標準,Ⅱ型及Ⅲ型為良性診斷標準,則超聲造影診斷良惡性腋淋巴結(jié)的敏感性、特異性、準確性分別為91.4%、73.3%、83.1%,而常規(guī)灰階超聲的診斷敏感性、特異性、準確性分別為68.6%、100%、83.1%。定量參數(shù)AT、TTP、PI在良惡性兩組間無顯著差異(P0.05)。結(jié)論:超聲造影對良惡性腋淋巴結(jié)鑒別診斷具有一定的價值,但與常規(guī)超聲相比,超聲造影并沒有明顯提高其診斷準確率。
[Abstract]:Objective: to study the features of benign and malignant axillary lymph nodes and to evaluate the value of contrast-enhanced ultrasonography in the diagnosis of breast cancer before operation.
