【摘要】：目的了解超聲內(nèi)鏡下2cm、符合間質(zhì)瘤影像學(xué)特點(diǎn)病變的隨訪情況,以選擇合理隨訪時(shí)間。方法回顧性分析超聲內(nèi)鏡下診斷2cm胃間質(zhì)瘤患者,記錄病變的部位、形態(tài)、大小、邊界、內(nèi)部回聲等特征。并經(jīng)超聲內(nèi)鏡隨訪至少1次,比較病變大小及內(nèi)部回聲等變化情況。結(jié)果共66例病例納入研究,根據(jù)隨訪時(shí)間記錄,隨訪半年20例,隨訪1年29例,隨訪2年26例,隨訪3年18例,隨訪4年9例,隨訪5年4例,其中24例患者隨訪1次。觀察超聲內(nèi)鏡下病變大小,2年隨訪患者內(nèi)鏡下病變面積40mm2,初始內(nèi)鏡下病變面積38mm2,病變面積有增大趨勢(shì),兩者比較有顯著性差異(P=0.0240.05)。病變位于胃底占78.8%(52/66),來(lái)源于固有肌層占95.5%(63/66),所有病例未見(jiàn)回聲異常改變及惡變表現(xiàn)。結(jié)論 2cm胃間質(zhì)瘤好發(fā)于胃底,主要來(lái)源于固有肌層,超聲內(nèi)鏡對(duì)2cm胃間質(zhì)瘤定期隨訪安全,有效,對(duì)于1.5cm的胃間質(zhì)瘤,建議可每2年復(fù)查一次。
[Abstract]:Objective to investigate the imaging features of stromal tumors (IST) under endoscopic ultrasonography (2cm) in order to select a reasonable follow-up time. Methods the location, shape, size, boundary and internal echo of 2cm gastric stromal tumors (GIST) diagnosed by endoscopic ultrasonography were analyzed retrospectively. Ultrasound endoscopy was followed up at least once to compare the changes of lesion size and internal echo. Results A total of 66 cases were included in the study. According to the records of follow-up, 20 cases were followed up for half a year, 29 cases for 1 year, 26 cases for 2 years, 18 cases for 3 years, 9 cases for 4 years and 4 cases for 5 years, among which 24 cases were followed up once. After 2 years follow-up, the lesion area was 40 mm 2 and 38 mm 2 respectively, and the lesion area increased with a significant difference (P < 0. 024 0. 05). The lesions were located in the fundus of the stomach in 78.8% (52 / 66) and from the muscular lamina propria in 95.5% (63 / 66). No abnormal echo or malignant changes were found in all cases. Conclusion 2cm gastric stromal tumors mainly originate from the lamina propria muscularis. EUS endoscopy is safe and effective for 2cm gastric stromal tumors. It is recommended that the gastric stromal tumors of 1.5cm should be reviewed every 2 years.
