【摘要】：目的：利用擴(kuò)散峰度成像技術(shù)探究輕度認(rèn)知功能障礙患者腦白質(zhì)微細(xì)結(jié)構(gòu)的改變,為其發(fā)病機(jī)制研究提供有價(jià)值的影像信息。方法：收集臨床上確診為輕度認(rèn)知功能障礙的患者20例,健康志愿者20例,行常規(guī)MRI平掃及DKI技術(shù)掃描,測(cè)量雙側(cè)額葉、頂葉、枕葉、顳葉、扣帶回后部及胼胝體壓部的FA值、MD值、MK值；針對(duì)有差異的感興趣區(qū),分析其與年齡、受教育年限的相關(guān)性。結(jié)果：病例組患者左側(cè)額葉、左側(cè)頂葉、雙側(cè)顳葉、雙側(cè)后扣帶回及雙側(cè)枕葉FA值均低于健康對(duì)照組,且左側(cè)頂葉FA值的變化在不同的年齡組有統(tǒng)計(jì)學(xué)差異。病例組患者左側(cè)額葉、右側(cè)頂葉、左側(cè)顳葉、左側(cè)后扣帶回及雙側(cè)枕葉MD值均高于健康對(duì)照組。病例組患者雙側(cè)顳葉、左側(cè)后扣帶回MK值均低于健康對(duì)照組,且左側(cè)扣帶回后部MK值的變化在男性與女性間有統(tǒng)計(jì)學(xué)差異。病例組患者雙側(cè)枕葉FA值與年齡呈負(fù)相關(guān)、與受教育年限呈正相關(guān),左側(cè)枕葉MD值與年齡呈正相關(guān)、與受教育年限呈負(fù)相關(guān),差異有統(tǒng)計(jì)學(xué)意義。結(jié)論：擴(kuò)散峰度成像技術(shù)為探究輕度認(rèn)知功能障礙腦白質(zhì)損害的發(fā)病機(jī)制提供了有價(jià)值的信息,其腦白質(zhì)損害的部位具有一定的選擇性。
[Abstract]:Aim: to explore the changes of white matter microstructures in patients with mild cognitive impairment by diffusion kurtosis imaging (DKI), and to provide valuable imaging information for the study of pathogenesis. Methods: twenty patients with mild cognitive impairment and 20 healthy volunteers were examined by conventional MRI scan and DKI technique. Bilateral frontal lobe, parietal lobe, occipital lobe and temporal lobe were measured. FA value and MK value of posterior cingulate gyrus and corpus callosum were analyzed. Results: the FA values of left frontal lobe, left parietal lobe, bilateral temporal lobe, bilateral posterior cingulate gyrus and bilateral occipital lobe in the case group were lower than those in the healthy control group, and the changes of FA value in the left parietal lobe were statistically different in different age groups. The MD values of left frontal lobe, right parietal lobe, left temporal lobe, left posterior cingulate gyrus and bilateral occipital lobe in the case group were higher than those in the healthy control group. The MK values of bilateral temporal lobe and left posterior cingulate gyrus in the case group were lower than those in the healthy control group, and the change of MK value in the left posterior cingulate gyrus was statistically different between male and female. The FA value of bilateral occipital lobe was negatively correlated with age, positively correlated with years of education, MD value of left occipital lobe was positively correlated with age and negatively correlated with years of education, the difference was statistically significant. Conclusion: diffusion kurtosis imaging provides valuable information for the study of the pathogenesis of mild cognitive impairment in the white matter, and the location of the white matter damage is selective.
