【摘要】：目的分析特殊類型乳腺惡性腫瘤的超聲表現(xiàn)與組織結(jié)構(gòu)的特點(diǎn),探討超聲對(duì)其術(shù)前的診斷價(jià)值。資料與方法回顧性分析經(jīng)術(shù)后病理證實(shí)的特殊類型乳腺惡性腫瘤7例患者的超聲表現(xiàn)和病理圖像的相關(guān)資料,觀察病變的組織結(jié)構(gòu)和乳腺各層解剖結(jié)構(gòu)及病變的聲像圖表現(xiàn),確定本組特殊類型乳腺惡性腫瘤的超聲分型,記錄病變的聲像圖特點(diǎn)。結(jié)果 7例患者中乳頭狀癌2例,黏液癌3例,淋巴瘤2例。乳頭狀癌腫塊與周圍組織界限清晰,侵犯間質(zhì),癌組織呈乳頭狀,致密;乳腺黏液癌見(jiàn)癌細(xì)胞漂浮在黏液湖中;原發(fā)乳腺淋巴瘤均為局灶性病變,為彌漫性大B細(xì)胞淋巴瘤。乳腺的超聲分型分別為脂肪型3例,腺纖維1型1例,腺纖維2型3例。2例乳頭狀癌呈低回聲,其中1例體積較大呈膨脹性生長(zhǎng);2例黏液癌為中低回聲,后方衰減不明顯,1例因鈣化伴聲影;淋巴瘤似無(wú)回聲,呈類囊樣。結(jié)論特殊類型乳腺惡性病變的組織結(jié)構(gòu)及聲像圖表現(xiàn)不同于常見(jiàn)的浸潤(rùn)性導(dǎo)管癌,提高對(duì)少見(jiàn)乳腺惡性腫瘤的認(rèn)識(shí)具有較高的臨床價(jià)值。
[Abstract]:Objective to analyze the features of ultrasonic findings and tissue structure of breast malignant tumors and to explore the diagnostic value of ultrasound before operation. Materials and methods Ultrasound findings and pathological images of 7 patients with special types of malignant breast tumors confirmed by postoperative pathology were retrospectively analyzed. The histological structure, anatomical structure of the breast and sonographic manifestations of the lesions were observed. To determine the ultrasonic classification of the special types of breast malignant tumors, and record the sonographic features of the lesions. Results there were 2 cases of papillary carcinoma, 3 cases of mucinous carcinoma and 2 cases of lymphoma. The boundary between the tumor and the surrounding tissue is clear, the invasion stroma is papillary and dense; the breast mucinous carcinoma shows the cancer cells floating in the mucous lake; the primary breast lymphoma is local lesion and diffuse large B cell lymphoma. There were 3 cases of fat type, 1 case of adenofibril type, 3 cases of adenofibril type 2 cases of papillary carcinoma and 2 cases of mucinous carcinoma. The posterior attenuation was not obvious in one case with calcification and acoustic shadow, and the lymphoma appeared to have no echo and was cystoid. Conclusion the histological structure and sonographic features of special breast malignant lesions are different from those of common invasive ductal carcinoma. It is of great clinical value to improve the understanding of rare breast malignant tumors.
