【摘要】：目的探討乳腺實性原位乳頭狀癌(SPC)的MR影像表現(xiàn)。方法回顧性分析5例經(jīng)手術(shù)病理證實的乳腺原位SPC患者的資料,觀察其術(shù)前常規(guī)MR平掃、動態(tài)增強(qiáng)掃描及DWI表現(xiàn)。結(jié)果原位SPC病灶T1WI脂肪抑制梯度回波(FSPGR)序列平掃表現(xiàn)為等和/或稍低信號,FSE T2WI、短時反轉(zhuǎn)恢復(fù)(STIR)序列平掃表現(xiàn)為等和/或稍高信號;動態(tài)增強(qiáng)MRI表現(xiàn)為邊界清楚的橢圓形或不規(guī)則形強(qiáng)化,病灶內(nèi)部強(qiáng)化均勻或不均勻。時間信號強(qiáng)度曲線早期快速上升,晚期呈平臺型或流出型。DWI病灶均表現(xiàn)為高信號,ADC值1.34×10~(-3)~1.96×10~(-3) mm~2/s。結(jié)論原位SPC的MRI表現(xiàn)具有一定特征,可為其診斷及與其他浸潤性乳腺癌的鑒別提供參考信息。
[Abstract]:Objective to investigate the MRI findings of solid in situ papillary carcinoma of the breast (SPC). Methods the data of 5 cases of breast in situ SPC confirmed by operation and pathology were analyzed retrospectively. The preoperative conventional Mr scan, dynamic contrast enhanced scan and DWI were observed. Results in situ SPC lesions, FSPGR showed isotropic and / or slightly hypointense FSE T2WI, and STIR showed isotropic and / or hyperintense signal on T 1WI. Dynamic contrast enhanced MRI showed elliptical or irregular enhancement with clear boundary and homogeneous or uneven enhancement within the lesion. The intensity curve of time signal increased rapidly in the early stage, and in the late stage, the lesions were flat or efflux. All the lesions showed high signal intensity (ADC) of 1.34 脳 10 ~ (-3) and 1.96 脳 10 ~ (-3) mm ~ (-3) / s. Conclusion MRI findings of in situ SPC have some characteristics, which can provide reference information for diagnosis and differential diagnosis of other invasive breast cancer.
