【摘要】：目的探討應用剪切波彈性成像(SWE)技術測定的頸部及腋窩淋巴結彈性模量值鑒別診斷正常與惡性淋巴結的價值。方法選取經(jīng)穿刺活檢或手術證實頸部及腋窩惡性淋巴結患者72例以及80名正常人,分別納入惡性及正常淺表淋巴結各100個,其中頸部淋巴結各60個,腋窩淋巴結各40個,于術前進行實時剪切波彈性成像,獲取淋巴結彈性模量值,分析比較頸部及腋窩正常、惡性淋巴結間彈性值的差異;以病理結果為"金標準",分別繪制并分析采用頸部、腋窩淋巴結彈性模量平均值診斷惡性淋巴結的ROC曲線,評價診斷效能。結果頸部正常淋巴結的彈性模量平均值為(18.94±5.20)kPa,惡性淋巴結為(25.48±5.36)kPa,腋窩正常淋巴結的彈性模量平均值為(11.68±2.27)kPa,惡性淋巴結為(19.71±3.93)kPa,頸部正常、惡性淋巴結的彈性模量平均值均分別高于腋窩正常、惡性淋巴結(P均0.01),同一部位(頸部、腋窩)的惡性淋巴結彈性平均值均高于正常淋巴結(P均0.01)。應用頸部淋巴結彈性模量平均值診斷惡性淋巴結,ROC曲線下面積(AUC)為0.81,以19.15kPa為診斷界值時,敏感度為93.3%,特異度為65.0%。應用腋窩淋巴結彈性模量平均值診斷惡性淋巴結,AUC為0.98,以13.95kPa為診斷界值時,敏感度為97.5%,特異度為87.50%。結論SWE彈性模量平均值可以為頸部及腋窩正常與惡性淋巴結的鑒別診斷提供依據(jù)。
[Abstract]:Objective to evaluate the value of elastic modulus of cervical and axillary lymph nodes in differential diagnosis of normal and malignant lymph nodes by using shear wave elastic imaging (SWE). Methods Seventy-two patients with cervical and axillary lymph nodes confirmed by puncture biopsy or operation and 80 normal controls were included in the study, including 100 malignant and normal superficial lymph nodes, including 60 cervical lymph nodes and 40 axillary lymph nodes, respectively. Real time shear wave elastic imaging was performed before operation to obtain the elastic modulus of lymph nodes, and to analyze and compare the difference of elastic modulus between the cervical and axillary lymph nodes, and to draw and analyze the neck according to the pathological results as "gold standard". The mean value of modulus of elasticity in axillary lymph nodes was evaluated by ROC curve for diagnosis of malignant lymph nodes. Results the average elastic modulus of normal lymph nodes was (18.94 鹵5.20) KPA, malignant lymph nodes (25.48 鹵5.36) KPA, axillary lymph nodes (11.68 鹵2.27) KPA and malignant lymph nodes (19.71 鹵3.93) KPA. The mean elastic modulus of malignant lymph nodes was higher than that of axillary lymph nodes (P0.01), and that of the same lymph nodes (neck, axillary) was higher than that of normal lymph nodes (P0.01). The area under the ROC curve (AUC) of cervical lymph nodes was 0.81.The sensitivity was 93.3and the specificity was 65.0 when 19.15kPa was used as the diagnostic threshold. The mean value of elastic modulus of axillary lymph nodes in diagnosis of malignant lymph nodes was 0.98. When the diagnostic threshold was 13.95 KPA, the sensitivity was 97.5 and the specificity was 87.50. Conclusion the average elastic modulus of SWE can provide evidence for differential diagnosis of normal and malignant lymph nodes in neck and axilla.
