【摘要】：目的:評價常規(guī)超聲結(jié)合超聲彈性成像在良惡性甲狀腺結(jié)節(jié)鑒別診斷中的應(yīng)用價值。方法 :72個甲狀腺結(jié)節(jié)術(shù)前均行常規(guī)超聲及超聲彈性成像檢查,將常規(guī)超聲及彈性成像結(jié)果與病理結(jié)果進(jìn)行比較。結(jié)果:常規(guī)超聲診斷甲狀腺結(jié)節(jié)的敏感度、特異度、準(zhǔn)確率分別為87.10%、82.93%、84.72%。超聲彈性成像圖中良性病變以0~Ⅱ級多見,而惡性病變以Ⅲ~Ⅳ級多見,以Ⅲ級以上(含Ⅲ級)作為判斷惡性的標(biāo)準(zhǔn),其診斷的敏感度、特異度、準(zhǔn)確率分別為90.32%、90.24%、90.28%。超聲彈性成像診斷的準(zhǔn)確率與常規(guī)超聲比較,差異沒有統(tǒng)計學(xué)意義(P0.05)。而常規(guī)超聲聯(lián)合超聲彈性成像,則其敏感度、特異度、準(zhǔn)確率分別為93.55%、97.56%、95.83%,其準(zhǔn)確率與常規(guī)超聲比較,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:常規(guī)超聲檢查聯(lián)合超聲彈性成像,可以提高良惡性甲狀腺結(jié)節(jié)鑒別診斷的敏感性,降低漏診率。
[Abstract]:Objective: to evaluate the value of conventional ultrasonography combined with elastography in differential diagnosis of benign and malignant thyroid nodules. Methods: 72 thyroid nodules were examined by conventional ultrasonography and elastography before operation. The results of conventional ultrasound and elastic imaging were compared with pathological findings. Results: the sensitivity, specificity and accuracy of conventional ultrasonography in the diagnosis of thyroid nodule were 87.1093 and 84.72, respectively.
