【摘要】：目的探討胃腔充盈程度對(duì)進(jìn)展期胃癌腫瘤厚度測(cè)量的影響及腫瘤厚度測(cè)量的臨床意義。方法收集進(jìn)展期胃癌患者38例(初診21例,非手術(shù)治療后復(fù)診17例),采用低張氣體充盈法,分別于平掃(充盈前)及增強(qiáng)靜脈期(充盈后)軸位圖像測(cè)量并比較胃腔充盈前、后各分區(qū)正常胃壁及腫瘤的厚度。同一觀察者于1個(gè)月后進(jìn)行再次測(cè)量,比較2次測(cè)量的一致性。結(jié)果各分區(qū)正常胃壁充盈前、后厚度的差異有統(tǒng)計(jì)學(xué)意義(P均0.001),胃體大彎厚度變化最大。胃腔充盈前后初診胃癌腫瘤厚度差異無統(tǒng)計(jì)學(xué)意義(P均0.05),而復(fù)診胃癌腫瘤厚度差異有統(tǒng)計(jì)學(xué)意義(P均0.05)。同一觀察者2次測(cè)量的腫瘤厚度一致性好。結(jié)論初診進(jìn)展期胃癌腫瘤厚度相對(duì)固定,可作為CT檢查的測(cè)量指標(biāo),復(fù)診時(shí)應(yīng)盡量使胃腔充盈程度與治療前一致,以準(zhǔn)確評(píng)價(jià)治療效果。
[Abstract]:Objective to investigate the effect of filling degree of gastric cavity on the measurement of tumor thickness in advanced gastric cancer and its clinical significance. Methods Thirty-eight patients with advanced gastric cancer (21 patients with advanced gastric cancer and 17 patients with non-operative treatment) were examined by hypotonic gas filling method. The axial images of plain scan (before filling) and enhanced vein (after filling) were measured and compared before and after gastric cavity filling. The thickness of normal gastric wall and tumor. The same observer remeasured one month later to compare the consistency of the two measurements. Results before filling the normal gastric wall, the difference of posterior thickness was statistically significant (P = 0.001), and the thickness of the great curvature of gastric body changed the most. There was no significant difference in tumor thickness before and after filling of gastric cavity (P < 0.05), but there was significant difference in tumor thickness between two groups (P < 0.05). The tumor thickness measured twice by the same observer was consistent. Conclusion the thickness of advanced gastric cancer is relatively fixed and can be used as a measure of CT examination. The degree of gastric cavity filling should be consistent with that before treatment in order to evaluate the therapeutic effect accurately.
