【摘要】：目的：采用脈沖多普勒超聲技術(shù)測(cè)量二尖瓣舒張期血流頻譜及頸部動(dòng)脈血流頻譜,探討不同年齡組高血壓患者左室舒張功能與頸動(dòng)脈血流動(dòng)力學(xué)特征之間的關(guān)系。方法：選擇高血壓患者608例,測(cè)量二尖瓣瓣口舒張期血流E峰及A峰,根據(jù)E峰及A峰的比值(E/A),按照E/A1和E/A1分成兩組,兩組內(nèi)按年齡分為青年組、中年組和老年組,使用高頻探頭測(cè)量頸總動(dòng)脈、頸內(nèi)動(dòng)脈及椎動(dòng)脈的收縮期峰速度(PSV)、舒張末期流速(EDV)、阻力指數(shù)(P1)、搏動(dòng)指數(shù)(RI)。各年齡組內(nèi)又使用spss17.0對(duì)組內(nèi),組間進(jìn)行統(tǒng)計(jì)學(xué)分析。兩組間頸動(dòng)脈血流動(dòng)力學(xué)參數(shù)根據(jù)分組采用獨(dú)立t檢驗(yàn)；各組二尖瓣舒張期血流指標(biāo)與頸動(dòng)脈血流動(dòng)力學(xué)參數(shù)采用相關(guān)性分析；多組間的血流動(dòng)力學(xué)指標(biāo)用單因素方差分析,方差不齊者用秩和檢驗(yàn)。結(jié)果：三個(gè)年齡組的高血壓患者組間比較：E/A1時(shí),頸總動(dòng)脈血流參數(shù)PSV、PI、RI隨年齡的增長(zhǎng)而降低。E/A1時(shí),頸總動(dòng)脈PSV、EDV和椎動(dòng)脈PSV均隨年齡的增長(zhǎng)而降低,頸總動(dòng)脈和頸內(nèi)動(dòng)脈的PI、RI隨年齡的增長(zhǎng)而增高。E/A1組與E/A1組相比,老年組內(nèi)頸總動(dòng)脈、頸內(nèi)動(dòng)脈及椎動(dòng)脈血流參數(shù)EDV下降,PI、RI升高。結(jié)論：左室舒張功能受損時(shí),頸部血管隨增齡血管順應(yīng)性減退,血流阻力增加。
[Abstract]:Objective: to investigate the relationship between left ventricular diastolic function and carotid artery hemodynamics in patients with hypertension of different ages by measuring mitral diastolic flow spectrum and carotid artery flow spectrum by pulsed Doppler ultrasound. Methods: the diastolic E and A peak of mitral valve orifice were measured in 608 patients with hypertension. According to the ratio of E / A peak and E / A peak, the patients were divided into two groups according to the ratio of E / A 1 and E / A 1. The two groups were divided into three groups according to their age: young group, middle age group and elderly group. The systolic peak velocities of common carotid artery, internal carotid artery and vertebral artery were measured by high frequency probe. Spss17.0 was also used in all age groups for statistical analysis. The carotid artery hemodynamic parameters of each group were analyzed by independent t test according to the grouping; the correlation analysis between mitral diastolic blood flow index and carotid artery hemodynamic parameter was used in each group; the hemodynamic parameters of multiple groups were analyzed by single factor variance analysis. Those with uneven variances were tested by rank sum test. Results: compared with the age group, the common carotid artery blood flow parameter PSV / PII RI decreased with the age. The common carotid artery PSV EDV and vertebral artery PSV decreased with the age of the patients with hypertension, and with the increase of age, the common carotid artery PSV EDV and the vertebral artery PSV decreased with the increase of age, and the blood flow parameters of the common carotid artery (PSV) and vertebral artery decreased with the increase of age. The PIRI of the common carotid artery and the internal carotid artery increased with age. Compared with the E / A1 group, the blood flow parameters of the common carotid artery, the internal carotid artery and the vertebral artery in the elderly group were decreased, and the blood flow parameters of the common carotid artery, the internal carotid artery and the vertebral artery in the elderly group were increased. Conclusion: when left ventricular diastolic function is impaired, the blood flow resistance increases with the decrease of vascular compliance.
