【摘要】：目的探討選擇性術(shù)前栓塞骶骨腫瘤動(dòng)脈的臨床應(yīng)用價(jià)值。方法對71例骶骨腫瘤切除術(shù)前患者行1~6次[平均(1.45±1.01)次]腫瘤動(dòng)脈栓塞。記錄患者術(shù)中和術(shù)后24h出血量和與栓塞相關(guān)的并發(fā)癥。單次栓塞與多次栓塞后手術(shù)切除患者的出血量比較采用獨(dú)立樣本t檢驗(yàn)。結(jié)果 54例患者在1次動(dòng)脈栓塞后行腫瘤切除術(shù);17例患者于2~6次栓塞后行腫瘤切除術(shù)。栓塞后手術(shù)患者術(shù)中和術(shù)后24h平均出血量(2457.75±1508.14)ml。單次栓塞后接受手術(shù)治療患者的平均出血量(2200.00±1350.47)ml,多次栓塞后手術(shù)患者的平均出血量為(3276.47±1724.29)ml,差異無統(tǒng)計(jì)學(xué)意義(t=1.588,P=0.212)。骨巨細(xì)胞瘤、脊索瘤、軟骨肉瘤和轉(zhuǎn)移瘤患者出血量差異無統(tǒng)計(jì)學(xué)意義(F=0.592,P=0.623)。結(jié)論骶骨腫瘤切除術(shù)前的動(dòng)脈栓塞可有效減少術(shù)中和術(shù)后出血,保證手術(shù)順利進(jìn)行。
[Abstract]:Objective to evaluate the clinical value of selective preoperative embolization of sacral tumor arteries. Methods Seventy-one patients with sacral neoplasms were treated with arterial embolization for 6 times (mean 1.45 鹵1.01). The volume of bleeding and the complications associated with embolism were recorded during and 24 hours after operation. Single embolization and multiple embolization were used to compare the amount of blood loss in patients undergoing surgical excision with independent sample t test. Results tumor excision was performed in 54 patients after one arterial embolization and tumor excision was performed in 17 patients after 2 times of embolization. The mean blood loss was 2457.75 鹵1508.14 ml during operation and 24 hours after embolization. The average volume of blood loss was 2200.00 鹵1350.47ml after single embolization, and 3276.47 鹵1724.29ml after multiple embolization. There was no significant difference between the two groups. There was no significant difference in blood loss between giant cell tumor of bone, chordoma, chondrosarcoma and metastatic tumor. Conclusion Arterial embolization before sacral tumor resection can effectively reduce intraoperative and postoperative bleeding and ensure smooth operation.
