【摘要】：目的探討大氣道黏液表皮樣癌的多層螺旋CT表現(xiàn)。方法搜集2005年至2012年間經(jīng)手術或支纖鏡病理證實的9例氣管、大支氣管黏液表皮樣癌患者資料,對病灶的多層螺旋CT特征進行評價。結(jié)果位于氣管、主支氣管的瘤灶3例,最大徑20 mm,表現(xiàn)為息肉樣軟組織密度影向管腔內(nèi)突出,無阻塞性改變。位于中間段支氣管、葉或段支氣管的瘤灶6例,均表現(xiàn)為球形病灶,最大徑20~68 mm;阻塞性肺不張1例,阻塞性肺炎6例。所有瘤灶均表現(xiàn)為表面光滑的均勻軟組織密度影,CT值約(54±16)HU,增強后均明顯均勻強化,強化前后差值為(64±14)HU。所有患者均未發(fā)現(xiàn)肺門、縱隔淋巴結(jié)增大,胸腔積液或胸膜增厚。結(jié)論大氣道黏液表皮樣癌好發(fā)于中青年,多位于主支氣管以下的大氣道,瘤灶強化明顯,常伴有阻塞性肺炎或肺不張。
[Abstract]:Objective to investigate the multi-slice spiral CT findings of mucoepidermoid carcinoma of the atmospheric tract. Methods from 2005 to 2012, 9 patients with mucoepidermoid carcinoma of trachea and bronchi proved by surgery or bronchoscopy were collected and the features of multi-slice spiral CT were evaluated. Results 3 cases were located in the trachea and the main bronchus with the maximum diameter of 20 mm. The results showed that the density of polypoid soft tissue protruded into the lumen without obstructive changes. 6 cases were located in the middle bronchus, lobes or segmental bronchi, all showed spherical lesions with a maximum diameter of 2068 mm, obstructive atelectasis in 1 case, obstructive pneumonia in 6 cases. The CT value of all tumor foci was about 54 鹵16 HUU, and the difference before and after enhancement was 64 鹵14 Hu. No hilar, mediastinal lymph node enlargement, pleural effusion or pleural thickening were found in all patients. Conclusion the mucoepidermoid carcinoma of the atmospheric tract is more common in young and middle-aged people, most of them are located in the atmospheric tract below the main bronchus, and the tumor foci are obviously enhanced, often accompanied by obstructive pneumonia or atelectasis.
