【摘要】：目的探討顱腦外傷(TBI)所致腦橋代謝物變化規(guī)律及其與預(yù)后的關(guān)系。方法對34例TBI患者行快速波譜成像(TSI),獲得NAA/Cr、Cho/Cr、Cho/NAA值,并與16例正常志愿者(對照組)進(jìn)行對比。將TBI組根據(jù)格拉斯哥預(yù)后評分分為預(yù)后良好、中等、差3組,比較不同組間代謝物水平差別。結(jié)果 TBI組34例腦橋NAA/Cr、Cho/Cr、Cho/NAA均值分別為2.51±0.73、2.56±0.67、1.05±0.29;對照組16例NAA/Cr、Cho/Cr、Cho/NAA均值分別為2.70±0.37、1.92±0.09、0.72±0.08。兩組間NAA/Cr均值差異無統(tǒng)計學(xué)意義(P=0.235),兩組間Cho/Cr、Cho/NAA均值差異有顯著統(tǒng)計學(xué)意義(P值均=0.000)。NAA/Cr在預(yù)后差組(2.15±0.56)與預(yù)后中等組(2.73±0.58)之間、預(yù)后差與預(yù)后良好組(2.98±0.90)間差異有統(tǒng)計學(xué)意義(P值分別為0.036、0.007)。Cho/Cr 3組之間差異無統(tǒng)計學(xué)意義。Cho/NAA在預(yù)后差組(1.22±0.31)與預(yù)后良好組(0.80±0.15)之間差異有統(tǒng)計學(xué)意義(P=0.001)。結(jié)論 TSI適合TBI腦橋MRS檢測,TBI患者腦橋Cho/Cr和Cho/NAA較正常人升高,而NAA/Cr和Cho/NAA比值對TBI預(yù)后判斷較有意義。
[Abstract]:Objective to investigate the changes of brain pons metabolites induced by TBI and their relationship with prognosis. Methods 34 patients with TBI underwent rapid spectral imaging with TSII to obtain NAA / Cro / CrChoP / NAA value, and compared with 16 normal volunteers (control group). According to Glasgow prognosis score, TBI group was divided into 3 groups with good prognosis, moderate prognosis and poor prognosis. Results in TBI group, the mean values of NAA / CrChoP / CrCho-NAA were 2.51 鹵0.73 鹵0.67 鹵1.05 鹵0.29 and 2.70 鹵0.37 鹵1.92 鹵0.090.72 鹵0.08 in the control group and 16 cases in the control group, respectively2.51 鹵0.73 鹵0.67 鹵0.67 鹵1.92 鹵0.090.72 鹵0.08, respectively, and the mean values of NAA / NAA in the control group were 2.70 鹵0.37 鹵1.92 鹵0.090.72 鹵0.08, respectively. There was no significant difference in the mean value of NAA / Cr between the two groups. There was significant difference in the mean value of NAA between the two groups (P < 0.000). NAA / Cr was between 2.15 鹵0.56 in the poor prognosis group and 2.73 鹵0.58 in the middle prognosis group (P < 0.05), and there was no significant difference between the two groups in the mean value of NAA / Cr in the poor prognosis group (2.15 鹵0.56) and the moderate group in the prognosis group (2.73 鹵0.58). The difference between poor prognosis group and good prognosis group (2.98 鹵0.90) was statistically significant (P = 0.036, P = 0.007, P < 0.05). There was no significant difference between the two groups (P = 0.80 鹵0.15). There was no significant difference between the poor prognosis group (1.22 鹵0.31) and the good prognosis group (0.80 鹵0.15). Conclusion TSI is suitable for the detection of pons in patients with TBI by Mrs, and the ratio of NAA / Cr and Cho/ NAA is higher than that of normal subjects, and the ratio of NAA / Cr and Cho/ NAA is more significant in predicting the prognosis of TBI.
