【摘要】：目的:通過對比釓特酸葡胺(Gd-DOTA)與釓噴酸葡胺(Gd-DTPA)在肝臟血管中的應(yīng)用,觀察兩種對比劑的MRI增強(qiáng)效能。方法:收集我院2016年11月-2017年2月期間參與肝臟血管增強(qiáng)受試者146例,其中Gd-DOTA組81例,男性67例,女性14例,年齡26~74歲,平均年齡52.53±10.73歲;GdDTPA組65例,男性41例,女性24例,年齡27~86歲,平均年齡52.16±10.89歲。所有受試者均在同臺機(jī)器進(jìn)行、應(yīng)用相同序列,采用Siemens Skyra 3.0T智多星DOT技術(shù)行肝臟MRI平掃及動態(tài)增強(qiáng)掃描,分別測量肝臟動態(tài)增強(qiáng)過程中蒙片、動脈期、門脈期、延遲期各期圖像的噪聲及肝動脈、門靜脈、腹主動脈的信號強(qiáng)度,計(jì)算得到信噪比、對比噪聲比。并對整體圖像質(zhì)量與增強(qiáng)效果進(jìn)行評分。結(jié)果:Gd-DOTA組腹主動脈在肝臟動態(tài)增強(qiáng)動脈期、門脈期、延遲期的SNR、CNR均高于Gd-DTPA組(P值分別為0.544、0.508;0.940、0.962;0.496、0.553),兩者差異無統(tǒng)計(jì)學(xué)意義;Gd-DOTA組門靜脈在肝臟動態(tài)增強(qiáng)動脈期、門脈期、延遲期的SNR、CNR值分別高于Gd-DTPA組(P值分別為0.494、0.383;0.766、0.766;0.535、0.654),兩者差異無統(tǒng)計(jì)學(xué)意義。Gd-DOTA組肝動脈在肝臟動態(tài)增強(qiáng)動脈期、門脈期、延遲期的SNR、CNR值分別高于Gd-DTPA組(P值分別為0.790、0.920;0.443、0.236;0.338、0.248),但兩者差異無統(tǒng)計(jì)學(xué)意義。兩組在圖像質(zhì)量與增強(qiáng)效果評分上無明顯統(tǒng)計(jì)學(xué)差異,P值均0.05。結(jié)論:Gd-DOTA在肝臟血管動態(tài)增強(qiáng)各期的信噪比、對比噪聲比均非劣于Gd-DTPA。
[Abstract]:Objective: To observe the MRI enhancement effect of two contrast agents by comparing the use of gadolinium acid glucamine (Gd-DOTA) and gadolinium acid glucamine (Gd-DTPA) in the liver vessels. Methods: 146 cases of liver vascular enhancement were collected in our hospital during February November 2016, including 81 cases in group Gd-DOTA, 67 men, 14 women, age 26~74 years, average year. Age 52.53 + 10.73 years, group GdDTPA 65 cases, male 41 cases, female 24 cases, age 27~86 years, average age 52.16 + 10.89 years old, all subjects were carried out in the same machine, using the same sequence, using Siemens Skyra 3.0T wisdom star DOT technique to perform liver MRI plain scan and dynamic enhanced scan, respectively, to measure the dynamic enhancement process of the liver and the arterial phase, respectively. The noise of the images and the signal intensity of the hepatic artery, the portal vein and the abdominal aorta in the portal period, the hepatic artery, the portal vein and the abdominal aorta were calculated and compared with the noise ratio. The overall image quality and enhancement were scored. Results: the abdominal aorta in the Gd-DOTA group was higher than the group Gd-DTPA (P value) in the dynamic enhanced arterial phase of the liver, the portal phase, the delay period, and the CNR. The difference was not statistically significant in 0.544,0.508; 0.940,0.962; 0.496,0.553). The portal vein in group Gd-DOTA was significantly higher than that in group Gd-DTPA (P value was 0.494,0.383; 0.766,0.766; 0.535,0.654) in the dynamic enhanced arterial phase of the liver, portal vein, and delayed phase (P value respectively), and there was no significant difference between the two groups of the hepatic artery in the liver. The SNR and CNR values of the strong artery, portal and delay period were higher than that of the Gd-DTPA group (P value 0.790,0.920; 0.443,0.236; 0.338,0.248), but there was no significant difference between the two groups. There was no significant difference between the two groups in the image quality and the enhancement effect score. The P values were all 0.05. knots: Gd-DOTA in the dynamic enhancement of the liver blood vessels. The ratio of noise to noise is not inferior to Gd-DTPA.
