【摘要】：目的探討母親圍孕期危險因素暴露與嬰幼兒先天性心臟病的發(fā)病風(fēng)險。方法病例選擇2012年3月1日~2013年10月1日在西京醫(yī)院經(jīng)超聲心動圖檢查確診為先天性心臟病的嬰幼兒,對照選擇為同期做超聲心動圖未發(fā)生先天性心臟病的嬰幼兒。對病例組和對照組父母親一般情況、母親孕期環(huán)境因素暴露情況進(jìn)行問卷調(diào)查。結(jié)果 435例先天性心臟病的嬰幼兒和574例未發(fā)生先天性心臟病的嬰幼兒的父母親基本情況及母親圍孕期相關(guān)危險因素分析結(jié)果顯示,母親懷孕早期服用藥物(OR=4.65,95%CI:2.84~7.66)、病毒感染(OR=3.15,95%CI:1.66~6.01)、房屋裝修(OR=1.87,95%CI:1.24~2.80)、染發(fā)燙發(fā)(OR=2.18,95%CI:1.37~3.49)、父母工作環(huán)境污染暴露(OR=2.88,95%CI:1.86~4.47)均是造成心血管畸形的獨(dú)立危險因素。隨著暴露因素的增多,先天性心臟病發(fā)病的危險程度顯著增加。當(dāng)孕期同時受到3種或3種以上危險因素的暴露時,嬰幼兒罹患先天性心臟病的危險性是單因素暴露的16.35倍。結(jié)論嬰幼兒先天性心臟病的發(fā)病與圍孕期危險因素的暴露存在相關(guān)性。
[Abstract]:Objective to explore the risk of maternal exposure during pregnancy and congenital heart disease in infants. Methods infants with congenital heart disease were selected from March 1, 2012 to October 1, 2013 in Xijing Hospital. The general condition of parents and maternal exposure to environmental factors during pregnancy were investigated in case group and control group. Results the parents' basic condition of 435 infants with congenital heart disease and 574 infants without congenital heart disease and the maternal risk factors during pregnancy were analyzed. In the early pregnancy, the mother took the drug ORO 4.65 / 95 CI: 2.847.66, the virus infected OR3.1595 / CI 1.66: 6.01, the house decoration 1.8795 CIW 1.24795 CIW1.242.80, the hair perm 2.1895 hair perm 1.373.49, the parents exposed to occupational pollution, OR2.88-95% CI1.864.47) were all independent risk factors for cardiovascular malformations. With the increase of exposure factors, the risk of congenital heart disease increased significantly. When exposed to three or more risk factors simultaneously during pregnancy, infants were 16.35 times more likely to develop congenital heart disease. Conclusion the incidence of congenital heart disease in infants is associated with exposure to risk factors during pregnancy.
