能譜CT在肌骨腫瘤良惡性鑒別中的價值

【摘要】:目的:通過對100例病理證實肌骨系統(tǒng)腫瘤所致軟組織腫塊的能譜CT分析,探討能譜CT在肌骨系統(tǒng)腫瘤良惡性鑒別中的應用價值。 材料與方法:對2011年10月-2013年4月在青島大學附屬醫(yī)院影像科接受MR掃描并顯示有軟組織腫塊的100例待手術肌骨系統(tǒng)腫瘤病例,應用寶石能譜CT(Discovery CT750, GE Heahhcare, Mliwaukee,美國)進行CT檢查。寶石CT掃描模式:CT能譜成像(Gemstone Speelral Imaing GSI)。使用能譜CT的圖像后處理技術,選擇CNR比選項,測試得最佳單能量值(keV),在單能量圖像上選取最佳keV。將感興趣區(qū)置于軟組織病變密度均勻的實性區(qū)域,避開壞死、囊變、出血及鈣化區(qū),由GSI Viewer軟件自動生成各ROI40keV-140keV能譜曲線。根據(jù)手術病理結果,將100例患者分為良性組(38例)、交界組(10例)、惡性組(49例)和脂肪瘤組(3例)(因脂肪瘤組的曲線呈負值弓背向上形與前三者明顯不同,予以單獨研究),計算每組平均斜率值:斜率(K40-80keV)=(HU40keV-HU80keV)/80-40。A-C組能譜曲線斜率統(tǒng)計學分析方法采用Kruskal-Wallis秩和檢驗及Mann-Whitney U方法,以P0.05為差異有統(tǒng)計學意義。 結果:惡性組的能譜曲線平均斜率為1.64±1.00,呈速降型,良性組的曲線平均斜率為0.75±0.30,呈緩慢下降型,交界組的斜率介于兩者之間為1.34±0.45。良性組斜率小于惡性組斜率(P0.05),良性組斜率小于交界性組斜率(P0.05)。3例脂肪瘤平均斜率為-2.00。 結論:CT能譜成像作為一項新的成像模式,在鑒別軟組織腫塊的良惡性方面有一定價值。負值弓背向上形曲線是含較多脂肪成分腫瘤的特征。
[Abstract]:Objective: to study the value of energy dispersive CT in differentiating benign and malignant tumors of myosseous system from 100 cases of soft tissue masses caused by pathologically proved musculoskeletal system tumors. Materials and methods: from October 2011 to April 2013, 100 cases of myosseous tumors with soft tissue masses were examined by CT(Discovery CT750, GE Heahcaree, Mliwaukee and Mliwaukee (USA) in the Department of Imaging, affiliated Hospital of Qingdao University. Gemstone Speelral Imaing GSI. By using the image post-processing technique of EDS CT and selecting the CNR ratio option, the best single energy value is tested and the best Kev is selected on the single energy image. The region of interest was placed in the solid area with uniform density of soft tissue lesions, avoiding necrosis, cysts, hemorrhage and calcification, and the ROI40keV-140keV energy spectrum curves were generated automatically by GSI Viewer software. According to the results of operation and pathology, 100 patients were divided into benign group (38 cases), borderline group (10 cases), malignant group (49 cases) and lipoma group (3 cases). The average slope value of each group was calculated by using Kruskal-Wallis rank sum test and Mann-Whitney U method. The slope was calculated by Kruskal-Wallis rank sum test and Mann-Whitney U method, and the difference was statistically significant (P 0.05) in HU40keV-HU80keVU / 80keV / / 80-40.A-C group. Results: the mean slope of the energy spectrum curve in malignant group was 1.64 鹵1.00, and that in benign group was 0.75 鹵0.30. The slope of boundary group was between 1.34 鹵0.45 and 0.75 鹵0.30, and that of borderline group was 1.34 鹵0.45. The mean slope of energy spectrum curve in malignant group was 1.64 鹵1.00, and that in benign group was 0.75 鹵0.30. The slope of benign group was smaller than that of malignant group (P 0.05), and that of benign group was lower than that of borderline group (P 0.05). The average slope of benign group was -2.00. Conclusion as a new imaging mode, the spectral imaging of CT has a certain value in differentiating benign and malignant soft tissue masses. The upward curve of the back of the negative arch is the characteristic of tumors with more fat components.
