【摘要】：目的 :評價IOTA簡易標(biāo)準(zhǔn)在鑒別卵巢良惡性腫瘤中的價值。資料與方法 :本研究所有患者均為本院2013年1月—2014年11月因卵巢腫物住院患者并均安排手術(shù)治療,所有患者均行超聲檢查并由第一作者根據(jù)IOTA簡易標(biāo)準(zhǔn)對腫瘤性質(zhì)進(jìn)行評定,所有患者均在1周內(nèi)完成手術(shù)。診斷的金標(biāo)準(zhǔn)是基于病理結(jié)果。計算IOTA簡易標(biāo)準(zhǔn)診斷的靈敏度、特異度、陽性預(yù)測值、陰性預(yù)測值、陽性似然比、陰性似然比。結(jié)果:總共252例患者中發(fā)現(xiàn)卵巢腫物,IOTA簡易標(biāo)準(zhǔn)適用于202例(80.2%)包括151例(74.8%)良性腫瘤,51例(25.2%)惡性腫瘤。簡易標(biāo)準(zhǔn)對50例(19.8%)無法判斷良惡性。在202例可以應(yīng)用此標(biāo)準(zhǔn)的腫瘤中,靈敏度為81.8%(95%CI:71.6%~92.0%),特異度為95.9%(95%CI:92.6%~99.2%)。結(jié)論:IOTA簡易標(biāo)準(zhǔn)對鑒別卵巢良惡性腫瘤方面具有較高價值,但是不能適用于所有卵巢腫瘤。
[Abstract]:Objective: to evaluate the value of IOTA standard in differentiating benign and malignant ovarian tumors.Materials and methods: all the patients in our institute were admitted to our hospital from January 2013 to November 2014 because of ovarian neoplasms and surgical treatment was arranged.All patients were examined by ultrasound and evaluated by the first author according to IOTA's simple criteria. All patients underwent surgery within one week.The gold criteria for diagnosis are based on pathological findings.The sensitivity, specificity, positive predictive value, negative predictive value, positive likelihood ratio and negative likelihood ratio of IOTA were calculated.Results: in a total of 252 patients, a simple IOTA standard was found to be suitable for 202 cases (80.2%) including 151 cases of benign tumor (51 cases with benign tumor) and 25.2% of malignant tumor.The simple standard for 50 cases (19.8%) can not be used to judge benign and malignant.In 202 cases of tumors that can be applied to this standard, the sensitivity is 81.8% and the sensitivity is 95%. The sensitivity is 92.0% and the specificity is 95.99.95% CI92.60.The sensitivity is 99.2%.ConclusionThe simple and simple standard of ion IOTA is valuable in differentiating benign and malignant ovarian tumors, but it is not applicable to all ovarian tumors.
