彌散加權(quán)成像定量評(píng)價(jià)非壓迫性神經(jīng)根炎微創(chuàng)治療價(jià)值的實(shí)驗(yàn)研究

【摘要】:目的:探討彌散加權(quán)成像技術(shù)能否對(duì)腰椎非壓迫性神經(jīng)根炎不同微創(chuàng)治療術(shù)后變化進(jìn)行定量評(píng)價(jià)。 材料與方法:中國(guó)小型豬12只CT導(dǎo)向下非壓迫性神經(jīng)根炎造模成功14天后,根據(jù)治療方法不同隨機(jī)分為4組,,每組3只實(shí)驗(yàn)動(dòng)物,包括:A臭氧治療組、B消炎液治療組、C膠原酶治療組、D對(duì)照組。分別于實(shí)驗(yàn)豬的L2~3、L3~4及L4~5雙側(cè)神經(jīng)根旁硬膜外注入相應(yīng)物質(zhì)進(jìn)行治療。術(shù)前常規(guī)行MRI冠狀位T2WI及DWI序列掃描。每組分別于術(shù)后第3天、7天及15天對(duì)實(shí)驗(yàn)豬進(jìn)行冠狀位T2WI及DWI掃描,掃描完成后處死1只實(shí)驗(yàn)動(dòng)物,取相應(yīng)雙側(cè)神經(jīng)根行CD8陽性T淋巴細(xì)胞免疫組化染色。分析各組于術(shù)前、術(shù)后3天、7天、15天神經(jīng)根ADC值變化及神經(jīng)根CD8陽性T淋巴細(xì)胞計(jì)數(shù)評(píng)分的變化;以及神經(jīng)根ADC值與神經(jīng)根CD8陽性T淋巴細(xì)胞計(jì)數(shù)評(píng)分的相關(guān)性。 結(jié)果:A、B、C、D組治療前、治療后3天、7天、15天神經(jīng)根平均ADC值均出現(xiàn)下降趨勢(shì)(P0.05);A、B、C、D組治療后3天、7天、15天神經(jīng)根CD8陽性T淋巴細(xì)胞計(jì)數(shù)評(píng)分均出現(xiàn)下降趨勢(shì)(P0.05)。A、B、C、D組神經(jīng)根ADC值與神經(jīng)根淋巴細(xì)胞計(jì)數(shù)定量評(píng)分均呈正相關(guān),相關(guān)系數(shù)分別為rs為0.806、0.916、0.889、0.644,P值均0.05;所有觀測(cè)淋巴細(xì)胞計(jì)數(shù)的神經(jīng)根ADC值與神經(jīng)根淋巴細(xì)胞計(jì)數(shù)定量評(píng)分呈正相關(guān),相關(guān)系數(shù)rs為0.905,P0.05。 結(jié)論:不同治療組均能較好的使神經(jīng)根炎得到改善,并且彌散加權(quán)成像技術(shù)能對(duì)腰椎非壓迫性神經(jīng)根炎微創(chuàng)治療術(shù)后變化進(jìn)行定量評(píng)價(jià)。
[Abstract]:Objective: to evaluate the effect of diffusion-weighted imaging (DWI) on the postoperative changes of lumbar noncompressed neuritis after different minimally invasive treatment.Materials and methods: twelve Chinese miniature pigs were randomly divided into 4 groups according to different treatment methods, 3 experimental animals in each group, after 14 days of successful modeling of non-compressed nerve root inflammation guided by CT.C collagenase treatment group and D control group.The corresponding substances were injected into the peridural space of the bilateral nerve roots of pig L2 and L4 5, respectively.MRI coronal T2WI and DWI sequences were performed before operation.In each group, coronal T2WI and DWI were performed on the 3rd day and 15th day after operation. After the scan, one animal was killed and the corresponding bilateral nerve roots were taken for CD8 positive T lymphocyte immunohistochemical staining.The changes of ADC value of nerve root and the score of CD8 positive T lymphocyte count in nerve root were analyzed before and 3 days after operation and 7 days after operation, and the correlation between ADC value of nerve root and CD8 positive T lymphocyte count score of nerve root was analyzed.Results before treatment, before treatment,The average ADC value of nerve root on the 3rd day, 7th day and 15th day after treatment all showed a downward trend. The ADC value of nerve root and the fine lymphoid of nerve root were decreased in group P0.05 and ANABU C + D on the 3rd day and 7th day after treatment and 15 days after treatment.The quantitative score of cell count was positively correlated.The correlation coefficients were r _ s 0.806 ~ 0.916 ~ 0.889U ~ (0.889) ~ 0.644m P = 0.05.The ADC value of nerve root was positively correlated with the quantitative score of nerve root lymphocyte count (r _ s = 0.905) (r _ s = 0.905) (P < 0.05), and the correlation coefficient (r _ s) was 0.905 (P ~ (0.05)).Conclusion: different treatment groups can improve nerve root inflammation, and diffusion weighted imaging can be used to quantitatively evaluate the changes of lumbar vertebrae non-compression neuritis after minimally invasive treatment.
