磁共振成像對(duì)膝關(guān)節(jié)移植軟骨生物力學(xué)指標(biāo)的前瞻性研究

【摘要】:目的探討磁共振T2mapping成像對(duì)基質(zhì)誘導(dǎo)的自體軟骨細(xì)胞移植(matrix-associated autologous chondrocyte transplantation, MACT)術(shù)后移植軟骨的評(píng)估價(jià)值。 方法9例接受MACT治療的膝關(guān)節(jié)軟骨損傷患者(16處軟骨損傷),分別在術(shù)后1、3、6、12個(gè)月進(jìn)行磁共振T2mapping檢查,測(cè)量各時(shí)間點(diǎn)移植區(qū)與正常區(qū)軟骨全層的T2值。采用配對(duì)t檢驗(yàn)比較各個(gè)時(shí)間點(diǎn)移植區(qū)與正常區(qū)軟骨T2值,采用單因素方差分析評(píng)估移植區(qū)軟骨T2值的縱向變化。 結(jié)果MACT術(shù)后1、3、6、12月移植區(qū)軟骨的T2值分別為82.00±17.73ms、71.56±12.33ms、57.30±6.13ms和49.74±5.15ms。術(shù)后各個(gè)時(shí)間點(diǎn)移植區(qū)T2值均大于正常區(qū),1、3月組有顯著差異(P0.05),6、12月組無(wú)顯著差異(P值分別為0.065、0.204)。術(shù)后1月和6月、1月和12月、3月和6月、3月和12月移植區(qū)T2值兩兩比較,差別均有統(tǒng)計(jì)學(xué)意義(P0.05),1月和3月、6月和12月差異無(wú)統(tǒng)計(jì)學(xué)差異(P分別為0.185、0.527);對(duì)于正常區(qū),各隨訪時(shí)間T2值無(wú)明顯統(tǒng)計(jì)學(xué)差異(P>0.05)。 結(jié)論磁共振T2mapping成像能夠無(wú)創(chuàng)、動(dòng)態(tài)地監(jiān)測(cè)移植軟骨的修復(fù)過(guò)程,可作為評(píng)估軟骨修復(fù)狀態(tài)的重要手段。 目的探討磁共振軟骨延遲增強(qiáng)成像(delayed gadolinium-enhanced MRI ofcartilage,dGEMRIC)對(duì)MACT后移植軟骨的評(píng)估價(jià)值。 方法9例接受MACT治療的膝關(guān)節(jié)軟骨損傷患者(16處軟骨損傷),于術(shù)后1、3、6、12個(gè)月分別行dGEMRIC,測(cè)量各時(shí)間點(diǎn)打藥前、后移植區(qū)及正常區(qū)軟骨全層的T1值,,計(jì)算R1(R1=1/T1post-1/T1pre)。采用配對(duì)t檢驗(yàn)比較各個(gè)時(shí)間點(diǎn)移植區(qū)與正常區(qū)軟骨的R1值,采用單因素方差分析評(píng)估移植區(qū)軟骨R1值的縱向變化。 結(jié)果MACT術(shù)后1、3、6、12月移植區(qū)的R1值分別為2.28±0.38s-1,1.93±0.31s-1,1.53±0.36s-1,1.08±0.15s-1。術(shù)后各時(shí)間點(diǎn)移植區(qū)R1值均明顯大于正常區(qū)(P<0.05),但隨著時(shí)間延長(zhǎng),移植區(qū)軟骨R1值逐漸接近正常軟骨。術(shù)后1月與6月、1月與12月、3月與6月、3月與12月、6月與12月移植區(qū)R1值兩兩比較差異均有統(tǒng)計(jì)學(xué)意義,(P<0.05);1月與3月移植區(qū)R1值差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.55);對(duì)于正常區(qū),各隨訪時(shí)間R1值無(wú)明顯統(tǒng)計(jì)學(xué)差異(P0.05)。 結(jié)論dGEMRIC技術(shù)可以定量反映移植軟骨生化成分修復(fù),對(duì)軟骨移植的隨訪具有重要的臨床應(yīng)用價(jià)值。
[Abstract]:Objective To evaluate the value of magnetic resonance T2mapping imaging ( MACT ) in the evaluation of matrix - associated chondrocytes transplantation ( MACT ) .
Methods Nine patients with knee joint cartilage injury treated with MACT were examined by magnetic resonance T2mapping at 1,3,6,12 months postoperatively . The T2 values were measured at each time point implantation area and normal area cartilage . Using paired t test , the value of T2 in each time point transplantation area and normal area was compared . A single factor analysis was used to evaluate the longitudinal changes of T2 value in the transplantation area .
Results The T2 values of 1 , 3 , 6 and 12 months after MACT were 82.00 鹵 17.73 ms , 71.56 鹵 12.33ms , 57.30 鹵 6.13ms and 49.74 鹵 5.15ms , respectively . There was no significant difference between the two groups ( P0.05 ) .
There was no significant difference in the follow - up time ( P > 0.05 ) .
Conclusion Magnetic resonance T2mapping can be used as an important means to assess the state of cartilage repair without invasive and dynamic monitoring of the repair process of transplanted cartilage .
Objective To evaluate the value of delayed enhancement - enhanced MRI of cartilage ( dgeRIC ) in the evaluation of cartilage after MACT .
Methods Nine patients with knee joint cartilage injury treated with MACT were measured at 1 , 3 , 6 and 12 months after operation , and the T1 values of the whole layer of cartilage were measured before and after each time point . The R1 values of each time point transplantation area and normal area cartilage were compared by paired t test . The longitudinal changes of the value of the cartilage R1 in the transplantation area were assessed by single factor analysis .
Results The R1 values of 1 , 3 , 6 and 12 months after MACT were 2 . 28 鹵 0 . 38s - 1 , 1 . 93 鹵 0 . 31 s - 1 , 1 . 53 鹵 0 . 36 s - 1 , 1.08 鹵 0 . 15 s - 1 .
There was no significant difference in the value of R1 between January and March ( P = 0.55 ) .
For the normal area , there was no significant difference in the value of R1 in the follow - up time ( P0.05 ) .
Conclusion The method can quantitatively reflect the repair of the biochemical components of the transplanted cartilage , and has important clinical application value for the follow - up of the cartilage transplantation .
