【摘要】：目的：對三陰性乳腺癌及HER-2過表達型乳腺癌的病理、磁共振動態(tài)增強特征及ADC值進行探討及分析,提高對兩者病理、磁共振特征的認識和判斷。方法：對經(jīng)免疫組化證實為三陰性乳腺癌的68名患者及55例HER-2過表達型乳腺癌患者的病理、MR動態(tài)增強特征進行分析,測量ADC值,同時進行統(tǒng)計學比較。結(jié)果：68例TNBC中浸潤性導管癌59例,髓樣癌5例,55例HER-2過表達型乳腺癌中浸潤性導管癌39例,導管原位癌13例,兩組乳腺癌在浸潤性導管癌、髓樣癌、導管原位癌比較有差異。TNBC在MRI上大多表現(xiàn)為類圓形、邊緣光整、環(huán)狀強化及相對高的ADC值,HER-2過表達性乳腺癌在MRI上大多表現(xiàn)為不規(guī)則形、邊緣毛刺、不均勻強化,相對低的ADC值。而在患者年齡,是否腫塊及TIC上,差異無統(tǒng)計學意義(P0.05)。結(jié)論：TNBC多為浸潤性導管癌及髓樣癌,HER-2過表達型乳腺癌多為浸潤性導管癌及導管原位癌,乳腺MR1動態(tài)增強形態(tài)學特征中病灶的形狀、邊緣、強化方式及ADC值對診斷TNBC及HER-2過表達性乳腺癌具有一定參考價值。
[Abstract]:Objective: to study and analyze the pathology, dynamic enhancement characteristics and ADC value of triple negative breast cancer and HER-2 overexpression breast cancer. Methods: dynamic contrast-enhanced MRI features of 68 patients with triple-negative breast cancer and 55 patients with HER-2 overexpression were analyzed and ADC values were measured. Results among 68 cases of TNBC, 59 cases of invasive ductal carcinoma, 5 cases of medullary carcinoma and 55 cases of HER-2 overexpression breast cancer, 39 cases of invasive ductal carcinoma, 13 cases of ductal carcinoma in situ, two groups of breast cancer in invasive ductal carcinoma, medullary carcinoma. Ductal carcinoma in situ most of the breast cancer on MRI appeared round, edge smooth, circular enhancement and relatively high ADC value of HER-2 over-expression breast cancer on MRI most of the irregular shape, edge burr, uneven enhancement. There was no significant difference in age, mass and TIC in patients with ADC. Conclusion the majority of invasive ductal carcinoma and medullary carcinoma are infiltrative ductal carcinoma and breast cancer with HER-2 overexpression in situ. The shape, edge, enhancement mode and ADC value of the lesions in the dynamic enhanced morphological features of breast MR1 have certain reference value for the diagnosis of TNBC and HER-2 overexpression breast cancer.
