【摘要】：目的探討首發(fā)未治療重性抑郁癥(MDD)患者經(jīng)針刺百會穴(GV20)后腦功能區(qū)的低頻振幅(ALFF)變化。方法將16例首發(fā)未治療MDD患者依據(jù)針刺真假情況,分別納為GV20組和假穴組,另將20名健康自愿者納為對照組。于針刺真、假穴前后分別行靜息態(tài)f MRI掃描,應(yīng)用ALFF方法分析受試者腦功能區(qū)變化情況。并采用統(tǒng)計(jì)學(xué)方法,檢驗(yàn)GV20組與另兩組間ALFF變化的差異。結(jié)果針刺前,GV20組ALFF值較對照組升高的區(qū)域主要位于右側(cè)前、中扣帶回、豆?fàn)詈�、楔前葉及兩側(cè)島葉,降低的區(qū)域主要位于兩側(cè)海馬、小腦及枕葉;針刺真、假穴后,與假穴組對比,GV20組右側(cè)前、中扣帶回、豆?fàn)詈思白髠?cè)島葉ALFF值降低,左側(cè)海馬、小腦ALFF值升高,與針刺前相應(yīng)異常腦區(qū)的變化相反。結(jié)論針刺GV20可能對MDD患者部分異常的腦功能區(qū)有調(diào)節(jié)作用�；趂 MRI技術(shù)的ALFF算法可為探索針刺治療MDD的中樞機(jī)制提供潛在的影像學(xué)證據(jù)。
[Abstract]:Objective to investigate the changes of low frequency amplitude of ALF in brain functional area after acupuncture at Baihui Point GV20 in patients with first-episode untreated major depressive disorder (MDD). Methods Sixteen patients without initial treatment of MDD were divided into GV20 group and sham point group according to the true and false situation of acupuncture. In addition, 20 healthy volunteers were taken as the control group. The rest f MRI scanning was performed before and after acupuncture at Zhenzhen and pseudo acupoints. The changes of brain functional areas were analyzed by ALFF method, and the changes of brain functional areas were analyzed by statistical method. Results compared with the control group, the increase of ALFF in the GV20 group was mainly located in the right anterior cingulate gyrus, the lentiform nucleus, the anterior cuneate lobe and the bilateral insular lobe, and the decreased area was mainly located in the bilateral hippocampus. The ALFF values of the right anterior, middle cingulate gyrus, lentiform nucleus and left insular lobe decreased, while the ALFF values of the left hippocampus and cerebellum increased in the cerebellum and occipital lobe after acupuncture. Conclusion Acupuncture GV20 may regulate some abnormal brain functional areas in patients with MDD. The ALFF algorithm based on f MRI technique can provide potential imaging evidence for exploring the central mechanism of acupuncture treatment of MDD.
