基于兔NAFLD模型的雙源CT及MRI肝臟脂肪定量研究

【摘要】:目的:探討雙源CT及MRI對肝臟脂肪含量測量的準(zhǔn)確性。方法:50只雄性新西蘭家兔隨機(jī)分為兩組,A組42只(實驗組),氋脂飼料喂養(yǎng);B組8只(對照組),普通飼料喂養(yǎng),共喂養(yǎng)16周。在開始喂養(yǎng)后的不同時期進(jìn)行雙源CT、MRI三維同相位和反相位及最小二程估算法迭代水脂分離(IDEAL-IQ)序列掃描。測量不同單能量圖像上肝臟的CT值、MRI同相位和反相位序列的肝臟脂肪含量(HFF)、IDEAL-IQ序列的質(zhì)子密度脂肪分量(PDFF)。后經(jīng)酸水解法獲得新鮮肝臟的脂肪含量。將單能量圖像上肝臟的CT值、HFF、PDFF與病理結(jié)果進(jìn)行Spearman相關(guān)性分析。結(jié)果:對照組正常肝臟及實驗組輕、中、重度脂肪肝的CT值、PDFF及HFF值差異均有統(tǒng)計學(xué)意義(P0.05)。CT值與病理結(jié)果呈負(fù)相關(guān),PDFF、HFF與病理結(jié)果呈正相關(guān)(P0.05)。雙源CT掃描80keV CT值與病理結(jié)果的相關(guān)性最好(r=-0.870);PDFF與病理結(jié)果的相關(guān)性(r=0.936)高于HFF(r=0.853)。結(jié)論:IDEAL-IQ序列所測的肝脂肪含量與肝實際脂肪含量的相關(guān)性高于同相位和反相位序列及雙源CT。
[Abstract]:Objective: to investigate the accuracy of dual-source CT and MRI in the measurement of hepatic fat content. Methods: fifty male New Zealand rabbits were randomly divided into two groups: group A (n = 42), group B (n = 8) fed with fat diet (control group, n = 8). A total of 16 weeks of feeding were performed. Three-dimensional in phase and inverse phase and least square estimation method were used to iterate the IDEAL-IQ sequence scanning of the liver at different stages after feeding. Ct values of liver on different single energy images were measured and MRI was in phase. The fat content of the fresh liver was obtained by acid hydrolysis method. The correlation between the CT value of the liver and pathological results was analyzed by Spearman. Results: the normal liver of the control group and the experimental group were mild, The difference of CT value and HFF value in severe fatty liver is statistically significant. There is a negative correlation between the CT value and pathological results. There is a positive correlation between HFF and pathological results. The correlation between CT value of 80 Kev and pathological results is the best. The correlation between PDFF and pathological results is the best. Conclusion the correlation between the liver fat content and the actual fat content in the liver measured by the WIDEAL-IQ sequence is higher than that in the same phase and inverse phase sequence and dual source CTs.
