【摘要】：目的：探討高頻超聲在肱骨外上髁炎中的應(yīng)用價(jià)值。 方法：對(duì)40名健康成年人及37名經(jīng)臨床診斷的肱骨外上髁炎患者雙側(cè)肘關(guān)節(jié)分別行高頻超聲檢查。測(cè)量各個(gè)伸肌總腱的厚度,對(duì)健康組及病例組的雙側(cè)肘關(guān)節(jié)分別進(jìn)行獨(dú)立樣本t檢驗(yàn),P0.05認(rèn)為差異有統(tǒng)計(jì)學(xué)意義。采用疼痛視覺模擬評(píng)分法(visual analogy scale, VAS)把肱骨外上髁炎疼痛分為輕、中和重度。分析超聲圖像,把伸肌總腱損傷分為輕、中和重度。對(duì)肱骨外上髁炎VAS疼痛級(jí)別和伸肌總腱損傷程度作Spearman等級(jí)相關(guān)分析,P0.05認(rèn)為有相關(guān)性。把伸肌總腱血流情況分為零、一、二、三級(jí)。對(duì)VAS疼痛級(jí)別和伸肌總腱血流程度作Spearman等級(jí)相關(guān)分析,P0.05認(rèn)為有相關(guān)性。 結(jié)果：①肱骨外上髁炎患者伸肌總腱在高頻超聲下表現(xiàn)主要有肌腱增厚,回聲減低或不均,肌腱內(nèi)部纖維不連續(xù),血流信號(hào)增加,周邊積液,病程較長(zhǎng)者還可合并肌腱內(nèi)鈣化,周邊骨質(zhì)改變等。②健康組雙肘伸肌總腱的肌腱厚度超聲測(cè)量值基本一致,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。③病例組患肘伸肌總腱的肌腱厚度超聲測(cè)量值大于健肘,差別有統(tǒng)計(jì)學(xué)意義(P0.05)。④肱骨外上髁炎VAS疼痛級(jí)別與伸肌總腱損傷程度呈正相關(guān)(P0.05)。⑤肱骨外上髁炎VAS疼痛級(jí)別與伸肌總腱血流程度呈正相關(guān)(P0.05) 結(jié)論：①高頻超聲可作為肱骨外上髁炎診斷的重要輔助檢查。②肱骨外上髁炎患者的疼痛程度與伸肌總腱損傷程度及血流豐富程度呈正相關(guān)。
[Abstract]:Objective: to evaluate the value of high-frequency ultrasound in the treatment of humeral epicondylitis. Methods: 40 healthy adults and 37 clinically diagnosed patients with epicondylitis of humerus were examined with high frequency ultrasound. T test of bilateral elbow joints in healthy group and case group (P0.05) showed that the difference was statistically significant. Visual analogy scale (vas) was used to classify the pain of lateral epicondylitis of humerus into mild, moderate and severe. The total tendon injury of extensor muscle was divided into light, moderate and severe. The Spearman grade correlation analysis of VAS pain grade and the degree of total tendon injury of extensor muscle of humerus epicondylitis was made. The blood flow of extensor tendon was divided into zero, one and two, respectively. VAS pain grade and total extensor tendon blood flow were analyzed by Spearman grade correlation analysis (P0.05). Results under high frequency ultrasound, the common tendon of extensor muscle in the supracondylitis supracondylitis of humerus showed mainly tendon thickening, low or uneven echo, discontinuous internal fibers, increased blood flow signal, peripheral effusion, and calcification of tendon in older patients. The thickness of tendon measured by ultrasound in the healthy group was the same as that in the healthy group. There was no significant difference in the thickness of tendon between the two cubitus extensor muscles, and there was no significant difference in the thickness of the tendon between the two cubitus extensor tendons and the normal elbow. There was a positive correlation between the VAS pain grade and the total tendon injury degree of extensor muscle. There was a positive correlation between the VAS pain level and the blood flow degree of the extensor tendon in the lateral epicondylitis of humerus (P0.055.The degree of VAS pain was positively correlated with the blood flow degree of the extensor tendon (P0.05)). Conclusion the high frequency ultrasound can be used as an important adjuvant examination for the diagnosis of extrahumeral epicondylitis .2 the degree of pain is positively correlated with the degree of injury of extensor tendon and the abundance of blood flow in patients with external epicondylitis of humerus.
