【摘要】：目的:評(píng)估64排螺旋CT對(duì)原發(fā)性頜面部成釉細(xì)胞瘤的診斷價(jià)值。方法:對(duì)112例經(jīng)手術(shù)病理證實(shí)的原發(fā)性頜面部成釉細(xì)胞瘤的術(shù)前CT圖像進(jìn)行回顧性分析。病理類型包括實(shí)體/多囊型70例、單囊型35例、促結(jié)締組織增生型6例和骨外/外周型1例。測量腫瘤增強(qiáng)前后興趣區(qū)(ROI)的CT值,研究該腫瘤的螺旋CT強(qiáng)化特征。結(jié)果:本組原發(fā)性頜面部成釉細(xì)胞瘤中囊性、囊實(shí)性、實(shí)性者分別為50、52、10例,分別占44.64%,46.43%,8.93%。多房狀75例(66.96%)。增強(qiáng)后囊壁、間隔和實(shí)性部分強(qiáng)化,可見壁結(jié)節(jié)樣強(qiáng)化,其中28例(84.85%)腫瘤強(qiáng)化程度達(dá)50%以上。111例骨內(nèi)腫瘤中97例邊緣有硬化線(87.39%),105例有明顯骨質(zhì)膨隆(94.59%),104例骨皮質(zhì)變薄或破壞、中斷(93.69%)。105例腫瘤與鄰牙有密切關(guān)系(93.75%)。結(jié)論:螺旋CT可清晰顯示原發(fā)性頜面部成釉細(xì)胞瘤的影像學(xué)特征,為指導(dǎo)臨床治療提供更多信息。
[Abstract]:Objective: to evaluate the value of 64-slice spiral CT in the diagnosis of primary maxillofacial ameloblastoma. Methods: the preoperative CT images of 112 cases of primary maxillofacial ameloblastoma proved by surgery and pathology were retrospectively analyzed. Type A includes 70 cases of solid / polycystic type, 35 cases of single cyst type, 6 cases of connective tissue hyperplasia type and 1 case of extraosseous / peripheral type. The CT value of ROI before and after enhancement was measured, and the spiral CT enhancement characteristics of the tumor were studied. Results: the cystic of primary maxillofacial ameloblastoma was found. There were 10 cases (44.64 / 46.43) of cysts and solid, respectively. There were 75 cases of multilocularis, including 66.96%. The posterior wall was enhanced, the septum and solid were partly enhanced, and the wall was nodular enhancement. The enhancement degree of the tumor was more than 50%. In 97 cases of intraosseous tumor, there were 87.39 sclerosing lines on the margin of 97 cases. There were 105 cases with obvious bony bulge and 94.59% bone cortex thinning or destruction. Conclusion: spiral CT can clearly display the imaging features of primary maxillofacial ameloblastoma and provide more information for clinical treatment.
