【摘要】：目的炎性腸病患者內臟痛的發(fā)生可能與中樞神經系統(tǒng)對疼痛的異常處理有關,文中旨在采用低頻振蕩振幅(ALFF)算法靜息態(tài)功能磁共振(rs-f MRI)研究炎性腸病患者腦功能改變的特點。方法收集2015年12月至2016年8月經南京醫(yī)科大學第二附屬醫(yī)院消化科炎性腸病患者27例(病例組)和21例健康志愿者(對照組)。高分辨率結構像和rs-f MRI數(shù)據,通過ALFF分析方法對比2組患者腦功能活動的改變,觀察病例組ALFF統(tǒng)計腦圖與臨床指標的相關性。結果與對照組比較,病例組內側額回、右側殼核、右側島葉、左中扣帶回、雙側輔助運動區(qū)等區(qū)域ALFF值降低;額中回、左側額上回、內側前額葉等區(qū)域的ALFF值升高(P0.05)。病例組頂下小葉、楔前葉、左中扣帶回ALFF值與血沉呈負相關(r=-0.537、-0.588、-0.588,P0.05);亦與病程、CRP呈負相關(P0.05)。內側額回ALFF值與CRP呈正相關(r=0.623,P=0.001)。結論炎性腸病患者存在多個腦區(qū)ALFF值的異常,主要位于與內臟疼痛、情緒的處理相關腦區(qū)。
[Abstract]:Objective the occurrence of visceral pain in patients with inflammatory bowel disease may be related to the abnormal treatment of pain in the central nervous system. The purpose of this paper is to study the changes of brain function in patients with inflammatory bowel disease by using the low frequency oscillatory amplitude shift (ALFF) algorithm. Methods from December 2015 to 2016, the second affiliated Hospital of Nanjing Medical University was collected to study the characteristics of brain function changes in patients with inflammatory bowel disease. 27 patients with inflammatory bowel disease (case group) and 21 healthy volunteers (control group) had high resolution structural imaging and rs-f MRI data. The changes of brain function in two groups were compared by ALFF analysis, and the correlation between ALFF and clinical indexes was observed. Results compared with the control group, the patients had medial frontal gyrus, right putamen nucleus, right insular lobe, left middle cingulate gyrus, and left middle cingulate gyrus. The ALFF value of bilateral auxiliary motor area decreased, the ALFF value of middle frontal gyrus, left superior frontal gyrus and medial prefrontal lobe increased (P0.05). The ALFF value of left cingulate gyrus was negatively correlated with erythrocyte sedimentation rate (ESR), and negatively correlated with the course of disease (P 0.05). The ALFF value of medial frontal gyrus was positively correlated with CRP. Conclusion there are many abnormal ALFF values in brain regions in patients with inflammatory bowel disease, mainly in visceral pain. Emotional processing related brain regions.
