肌萎縮側(cè)索硬化癥認(rèn)知損害的磁共振成像研究

【摘要】:肌萎縮側(cè)索硬化癥是一種累及上下運(yùn)動(dòng)神經(jīng)元的進(jìn)行性變性病,但也可出現(xiàn)運(yùn)動(dòng)系統(tǒng)外受累的表現(xiàn),如執(zhí)行功能障礙等認(rèn)知受損表現(xiàn)。本研究應(yīng)用靜息態(tài)功能磁共振成像,采用低頻振幅(low frequency fluctuation, ALFF)、局部一致性(regional homogeneity, ReHo)、功能連接性(functional connectivity, FC)分析方法,對3例伴有認(rèn)知損害的ALS患者(ALSi組)和8例不伴認(rèn)知損害的ALS患者(ALSu組)的默認(rèn)網(wǎng)絡(luò)(default mode network, DMN)、感覺運(yùn)動(dòng)網(wǎng)絡(luò)(sensorimotor network,SMN)和左側(cè)額頂網(wǎng)絡(luò)(frontoparietal network, FPN)的功能活動(dòng)性和功能連接性進(jìn)行了組間比較。結(jié)果顯示,與ALSu組患者相比,ALSi組患者在SMN和左側(cè)FPN相應(yīng)腦區(qū)的ALFF增加,SMN在雙側(cè)旁中央小葉與自身的FC增加,SMN的雙側(cè)旁中央小葉與左側(cè)FPN的左額下回三角部/眶部之間FC增加,左側(cè)FPN的左額下回三角部/眶部與DMN的左側(cè)角回之間FC減弱。這種功能活動(dòng)性及功能連接性的改變可能是左側(cè)FPN對SMN運(yùn)動(dòng)功能的代償,同時(shí)可能也是引起認(rèn)知受損的原因。
[Abstract]:Amyotrophic lateral sclerosis is a progressive sexually transmitted disease involving the upper and lower motor neurons, but it can also show involvement outside the motor system. In this study, resting functional magnetic resonance imaging was performed with low-frequency amplitude low frequency fluctuation. ALFFN, regional homogeneity (ReHoA). Functional connectivity analysis method. Default network for 3 ALS patients with cognitive impairment and 8 ALS patients without cognitive impairment. Default mode network. DMNs, sensorimotor networksSMNs (sensorimotor network) and frontoparietal network (left frontopia network). The results showed that compared with the ALSu group, the ALFF in the corresponding brain region of SMN and left FPN was increased in the SMN group and the left FPN group. SMN increased between the bilateral central lobules and their own FC between the bilateral paracentral lobules and the left inferior frontal gyrus / orbital region of left FPN. FC between the left inferior frontal / orbital gyrus and the left angular gyrus of DMN was weakened in left FPN. This change of functional activity and functional connectivity may be the compensatory effect of left FPN on the motor function of SMN. It may also be the cause of cognitive impairment.
