【摘要】：目的建立胎兒右心系統(tǒng)多項參數(shù)的Z值評分正常參考值范圍,定量評估胎兒右心系統(tǒng)功能。方法應(yīng)用超聲心動圖對2 410例孕齡為22~38周的胎兒進行檢測,根據(jù)停經(jīng)史計算胎兒孕周(GA),測量股骨長徑(FL)、雙頂徑(BPD)。通過標(biāo)準(zhǔn)四腔心切面測量心臟收縮末期右房橫徑(RATD)、長徑(RALD)、肺動脈瓣環(huán)內(nèi)徑(PA);舒張末期右室橫徑(RVTD)、長徑(RVLD)。以GA、FL及BPD為自變量,右心系統(tǒng)參數(shù)為應(yīng)變量建立最佳模型,并進行標(biāo)準(zhǔn)差(SD)的校正,運用公式計算Z值評分。結(jié)果胎兒生物學(xué)生長參數(shù)GA、FL、BPD與RATD、RALD、RVTD、RVLD、PA呈良好的直線相關(guān)關(guān)系。采用絕對殘差加權(quán)線性回歸消除SD的異方差性,建立SD直線回歸模型,根據(jù)方程得出右心系統(tǒng)參數(shù)的Z值評分。結(jié)論應(yīng)用統(tǒng)計學(xué)方法,建立了通過GA、FL、BPD計算RATD、RALD、RVTD、RVLD、PA的Z值評分正常參考范圍,為胎兒右心系統(tǒng)功能的定量評估提供更加精準(zhǔn)的參考標(biāo)準(zhǔn)。
[Abstract]:Objective to establish the normal reference range of Z value score for multiple parameters of fetal right heart system. Methods 2410 fetuses of 22 ~ 38 weeks gestational age were measured by echocardiography, and the gestational weeks (GAA) were calculated according to the history of menopause. The right atrial transverse diameter (RATD), the long diameter of RALDD, the inner diameter of pulmonary annulus were measured by standard four-chamber central section. The end diastolic right ventricular transverse diameter (RVTD) and long diameter RV LDD were used to establish the best model with GAFL and BPD as independent variables and the parameters of right heart system as dependent variables, and the calibration of standard deviation (SDD) was carried out. Results the parameters of fetal biological growth, GAFL-BPD and RATDX RALDD RVTDX RVLD, were calculated by the formula. The absolute residual weighted linear regression was used to eliminate the heteroscedasticity of SD and the SD linear regression model was established. According to the equation, the Z score of the parameters of the right heart system was obtained. Conclusion the RATDX RALDD RVTDX RVLD was established by using the statistical method. The normal reference range of Z score of PA provides a more accurate reference standard for quantitative evaluation of fetal right ventricular system function.
