qPCR檢測(cè)結(jié)核桿菌在肺和胸膜上皮樣肉芽腫性疾病診斷中的意義

【摘要】:目的探討熒光定量聚合酶鏈反應(yīng)(Real-time polymerase chain reaction,q PCR)法在診斷及鑒別診斷肺和胸膜結(jié)核病和其他肉芽腫性疾病中的意義。方法收集經(jīng)病理學(xué)檢查發(fā)現(xiàn)上皮樣肉芽腫性病變的肺和胸膜活檢的石蠟包埋組織142例,應(yīng)用q PCR法檢測(cè)結(jié)核分枝桿菌DNA,同時(shí)行抗酸染色,并與臨床資料對(duì)比進(jìn)行回顧性分析。結(jié)果在142例肺胸膜活檢標(biāo)本中,60例臨床診斷確診為結(jié)核病。采用q PCR法檢測(cè)結(jié)核分枝桿菌DNA陽(yáng)性者58例,敏感性為96.67%,特異性為100%;抗酸染色陽(yáng)性者17例,敏感性為28.33%,特異性為97.56%,差異有統(tǒng)計(jì)學(xué)意義(χ~2=21.47,P0.001)。結(jié)論結(jié)核分枝桿菌核酸檢測(cè)有助于在石蠟包埋的肺和胸膜小活檢組織中快速、準(zhǔn)確的診斷結(jié)核病,可用于疑似結(jié)核的肉芽腫性病變的鑒別診斷。
[Abstract]:Objective to investigate the significance of real-time polymerase chain reactionqPCR in the diagnosis and differential diagnosis of pulmonary and pleural tuberculosis and other granulomatous diseases. Methods 142 cases of lung and pleural biopsy paraffin embedded tissues with epithelial granulomatous lesions were collected. The DNA of Mycobacterium tuberculosis was detected by qPCR and acid fast staining was performed, and the clinical data were analyzed retrospectively. Results 60 of 142 pulmonary pleural biopsy specimens were diagnosed as tuberculosis. The sensitivity was 96.677.The specificity was 100 and the sensitivity was 28.33 and the specificity was 97.56 (蠂 ~ 2 / 2) in 17 cases of positive acid-fast staining (蠂 ~ (2 +) 21.47 ~ (7) P 0.001) by using qPCR method to detect the DNA positive of Mycobacterium tuberculosis in 58 cases (蠂 ~ (2) 2 + 21.47) (P ~ (0.001). Conclusion Detection of Mycobacterium tuberculosis nucleic acid is helpful for rapid and accurate diagnosis of tuberculosis in paraffin-embedded lung and pleural small biopsy tissues, and can be used for differential diagnosis of granulomatous lesions suspected of tuberculosis.
