【摘要】：腦卒中是現(xiàn)代社會(huì)發(fā)病率最高的疾病之一，卒中后大腦的局部損傷，不僅導(dǎo)致可測量的組織學(xué)改變，還引起神經(jīng)心理學(xué)方面的變化。神經(jīng)組織損傷可造成患者軀體感覺、運(yùn)動(dòng)功能障礙和嚴(yán)重的心理功能失調(diào)。同時(shí)，軀體功能受損還可引起和(／或)加重心理功能的紊亂。 記憶功能的損害是腦卒中患者神經(jīng)心理學(xué)最明顯的變化之一，由記憶功能損傷引發(fā)患者情緒、認(rèn)知、行為的改變，后者又可加重記憶功能障礙，從而形成惡性循環(huán)，，嚴(yán)重?fù)p害患者的社會(huì)功能與生活質(zhì)量。 腦卒中的現(xiàn)代康復(fù)理論提倡早期肢體康復(fù)和心理功能康復(fù)。加快腦卒中患者的記憶力恢復(fù)和提高記憶力、改善不良情緒，最大限度地提高其社會(huì)再適應(yīng)能力及生活質(zhì)量是現(xiàn)代臨床心理學(xué)和康復(fù)醫(yī)學(xué)研究的熱點(diǎn)問題。雖然對(duì)卒中后患者的記憶和情緒研究較多，但對(duì)于腦卒中急性期患者的研究相對(duì)較少。 本課題旨在研究：1．腦卒中急性期患者記憶的變化規(guī)律；2．腦卒中急性期患者內(nèi)隱記憶與外顯記憶的損傷特征；3．腦卒中急性期患者情緒障礙的特點(diǎn)；4．腦卒中急性期患者情緒與記憶的關(guān)系；5．不同卒中部位外顯記憶與內(nèi)隱記憶改變的特點(diǎn)以揭示不同腦區(qū)域?qū)τ谟洃浀呢暙I(xiàn)；6．腦卒中急性期患者記憶、情緒與不同基礎(chǔ)性疾病的相關(guān)性。 本研究采用成組設(shè)計(jì)的對(duì)照研究，選用2005年3月至10月于山東省齊魯醫(yī)院神經(jīng)內(nèi)科住院的首次腦卒中患者(包括腦出血和腦梗塞)30名和年齡、受教育程度與患者組相匹配的正常老年人30名作為對(duì)照組。首先用長谷川癡呆量表進(jìn)行篩查，排除癡呆患者，對(duì)被試進(jìn)行一般資料的調(diào)查；由被試獨(dú)立完成SAS、SDS；由調(diào)查者對(duì)被試進(jìn)行漢密爾頓抑郁評(píng)分；利用臨床記憶量表進(jìn)行記憶力測試；后利用自制內(nèi)隱記憶效應(yīng)量表進(jìn)行內(nèi)隱記憶測試。 研究結(jié)果顯示：1．患者記憶商、臨床記憶量表分及分量表分均明顯低于國
[Abstract]:Cerebral apoplexy is one of the most common diseases in modern society. Local brain injury after stroke not only leads to measurable histological changes but also changes in neuropsychology. Nerve tissue injury can cause somatosensory, motor dysfunction and severe psychological dysfunction. At the same time, the impairment of somatic function can also cause and / or aggravate the disorder of psychological function. Memory impairment is one of the most obvious changes in neuropsychology in stroke patients. Memory impairment can cause emotional, cognitive and behavioral changes in stroke patients, and the latter can aggravate memory dysfunction, thus forming a vicious circle. The social function and quality of life of the patients were seriously damaged. The modern rehabilitation theory of cerebral apoplexy advocates early limb rehabilitation and psychological function rehabilitation. It is a hot issue in modern clinical psychology and rehabilitation medicine to speed up the recovery of memory and improve memory, improve bad mood and improve the social readability and quality of life of stroke patients to the maximum extent. Although there are many studies on memory and emotion of post-stroke patients, there are relatively few studies on acute stroke patients. The purpose of this study is to study: 1. The changes of memory in patients with acute stroke; 2. The impairment characteristics of implicit memory and explicit memory in patients with acute stroke; 3. The characteristics of emotional disorder in patients with acute stroke; 4. The relationship between emotion and memory in patients with acute stroke, 5. The changes of explicit memory and implicit memory in different stroke areas to reveal the contribution of different brain regions to memory. 6. The correlation between memory, emotion and different basic diseases in patients with acute cerebral apoplexy. In this study, 30 patients with first stroke (including cerebral hemorrhage and cerebral infarction) who were hospitalized in Department of Neurology, Qilu Hospital, Shandong Province from March to October 2005 were selected as control study. Thirty normal elderly persons with education level matched with the patient group served as control group. First, the Hasegawa dementia scale was used to screen the dementia patients, and the subjects were investigated with general data, and the subjects were independently completed by SAS,SDS; and Hamilton depression scores were assessed by the subjects. Clinical memory scale was used for memory test, and self-made implicit memory effect scale was used for implicit memory test. The results of the study showed that: 1. The patients' memory quotient, clinical memory scale and subscale scores were significantly lower than those of China.
