兔肝外膽道內(nèi)臟感覺上行傳導(dǎo)途徑的實(shí)驗(yàn)研究

【摘要】: 目的:本實(shí)驗(yàn)通過造模觀察與內(nèi)臟痛相關(guān)核團(tuán)的表達(dá),探討內(nèi)臟感覺傳導(dǎo)途徑。方法:取新西蘭長(zhǎng)耳兔32只,隨機(jī)分為8組,每組4只。分別為兩組假手術(shù)組(Ⅰ-A組、Ⅰ-B組)、去膽囊組(Ⅱ組)、去膽囊膽管注射大腸桿菌組(Ⅲ組)、膽囊內(nèi)注射大腸桿菌組(Ⅳ組)、膽管擴(kuò)張組(Ⅴ組)、擴(kuò)張Oddis括約肌組(Ⅵ組)、擴(kuò)張Oddis括約肌并切斷膈神經(jīng)組(Ⅶ組)。造模后Ⅱ、Ⅲ、Ⅳ組三天將兔處死;Ⅴ、Ⅵ、Ⅶ組2小時(shí)將兔處死,取脊髓、腦干以及間腦作連續(xù)切片后,采用HE染色光鏡定位,運(yùn)用免疫組化技術(shù)顯示相關(guān)核團(tuán),并進(jìn)行統(tǒng)計(jì)學(xué)處理。結(jié)果:1、去膽囊組(Ⅱ組)NOS在脊髓后角(C_(5-7))和孤束核中表達(dá)具有統(tǒng)計(jì)學(xué)意義(p<0.05);2、去膽囊膽管注射大腸桿菌組(Ⅲ組)NOS在脊髓(C_(5-7)、T_(6-9))后角、薄束核、楔束核、丘腦腹后外側(cè)核、孤束核中表達(dá)具有統(tǒng)計(jì)學(xué)意義(p<0.05);3、膽囊內(nèi)注射大腸桿菌組(Ⅳ組)NOS在脊髓(C_(5-7)、T_(6-9))后角、薄束核、楔束核、丘腦腹后外側(cè)核、孤束核表達(dá)具有統(tǒng)計(jì)學(xué)意義(p<0.05);4、膽管擴(kuò)張組(Ⅴ組)C-fos在脊髓(C_(5-7)、T_(6-9))后角、薄束核、楔束核、孤束核、丘腦腹后外側(cè)核表達(dá)具有統(tǒng)計(jì)學(xué)意義(p<0.05);5、擴(kuò)張Oddis括約肌組(Ⅵ組)C-fos在脊髓后角(C_(5-7))和孤束核中表達(dá)具有統(tǒng)計(jì)學(xué)意義(p<0.05);6、擴(kuò)張Oddis括約肌并切斷膈神經(jīng)組(Ⅶ組)C-fos在脊髓(C_(5-7))后角表達(dá)明顯降低不具有統(tǒng)計(jì)學(xué)意義(p>0.05)。結(jié)論:1、膽囊和膽管引起的疼痛均與脊髓(C_(5-7)、T_(6-9))后角、薄束核、楔束核、丘腦腹后外側(cè)核、孤束核有關(guān),推測(cè)由交感和迷走神經(jīng)雙重支配2、Oddis括約肌引起的疼痛與孤束核有關(guān),推測(cè)由迷走神經(jīng)傳入3、肝外膽道的感覺傳入不僅包括交感和副交感神經(jīng),還有右膈神經(jīng)。
