鼠聲帶損傷動物模型的建立及修復早期聲帶Ⅲ型膠原和纖維粘連蛋白的表達

【摘要】: 背景和目的:繼發(fā)于感染、損傷和外科手術(shù)的聲帶瘢痕(vocal fold scar)是聲帶受損后引起音質(zhì)改變的主要原因。研究提示:聲帶瘢痕一般發(fā)生在聲帶的固有層(lamina propria,LP)。聲帶固有層主要由纖維蛋白、糖蛋白和蛋白多糖等多種細胞外基質(zhì)(extracellular matrix,ECM)組成,細胞外基質(zhì)組成物的含量、性質(zhì)及各組成物之間的相互作用對正常聲帶生物機械特性的維持具有重要意義。本實驗使用大鼠構(gòu)建聲帶機械損傷動物模型,運用免疫組化染色,觀察修復早期不同時間聲帶固有層中Ⅲ型膠原(collagenⅢ)和纖維粘連蛋白(fibronectin,F(xiàn)N)的表達及分布變化規(guī)律。 材料和方法:4—6個月的SD大鼠45只,雌雄不限,平均體重約420g。實驗組40只在直徑1.9mm鼻內(nèi)窺鏡的輔助下,行雙側(cè)聲帶表面被覆層剝脫術(shù),術(shù)后死亡4只,將存活的36只隨機分為A、B、C、D4組,平均每組9只,于術(shù)后不同時間(3天、5天、7天、14天)獲取全喉標本。5只為正常對照E組,在實驗之初即處死獲取全喉標本。所有標本經(jīng)10%中性甲醛固定,石蠟包埋,4μm連續(xù)切片,用免疫組化染色檢測Ⅲ型膠原和纖維粘連蛋白的表達,采用HPIAS-100高清晰度彩色病理圖文分析系統(tǒng)將染色不同深淺度轉(zhuǎn)化為灰度數(shù)值,作定量檢測。將上述四個實驗組所得灰度數(shù)值及對照組進行統(tǒng)計學分析,,統(tǒng)計學采用單因素方差分析中的Dunnett-t,SNK-q檢驗,所有數(shù)據(jù)輸入SPSS13.0統(tǒng)計學軟件進行處理。 結(jié)果:1.HE染色:術(shù)后3天的標本HE染色顯示正常聲帶粘膜上皮及固有層缺失,新鮮肉芽組織形成并有少量膠原纖維表達;5天受損區(qū)域上皮爬行并增生形成復層結(jié)構(gòu),新鮮肉芽組織、膠原大量形成;7天上皮爬行基本完成,LP內(nèi)再生膠原組織形成粗大束狀結(jié)構(gòu);14天損傷區(qū)上皮變薄呈單層上皮結(jié)構(gòu),LP內(nèi)膠原組織重排。 2.免疫組化染色:對照組中Ⅲ型膠原主要分布于固有層淺層和深層。纖維粘連蛋白主要分布于基底膜區(qū)域,少量表達于固有層淺層。實驗組Ⅲ型膠原和纖維粘連蛋白14天內(nèi)各時間段兩者表達都與對照組有顯著性差異(P<0.05),但各實驗組之間表達無差異(P>0.05)。表達范圍擴大至整個固有層,表達不均勻,呈斑片狀,固有層內(nèi)細胞增多。且Ⅲ型膠原和纖維粘連蛋白的表達呈正相關(γ=0.667,P=0.001)。 結(jié)論:1.盡管大鼠聲帶較小,但在內(nèi)窺鏡輔助下能夠準確損傷大鼠聲帶被覆層,構(gòu)建大鼠聲帶損傷動物模型,觀察大鼠聲帶瘢痕形成過程,為聲帶損傷及聲帶瘢痕的形成過程的深入研究提供有利條件。 2.急性聲帶損傷早期Ⅲ型膠原和纖維粘連蛋白表達均增高,分布區(qū)域至固有層全層;且二者在固有層中的表達有一定相關性,說明纖維粘連蛋白在聲帶固有層中導向膠原沉積的模板作用。Ⅲ型膠原14天內(nèi)維持高水平恒定表達。
