文章摘要:目的 觀察大鼠佐劑性關(guān)節(jié)炎(adjuvant rheumatoid arthritis,AA)病程不同階段的脾臟T細(xì)胞亞群及其重要器官的病理學(xué)變化,探討病程不同的階段AA大鼠T細(xì)胞亞群變化的規(guī)律及局部組織和全身重要器官的病理學(xué)變化特點(diǎn)。方法 選用20只♂Lewis大鼠,完全弗氏佐劑(CFA)誘導(dǎo)AA模型,造模后d7、d10、d13、d16、d19、d22進(jìn)行全身評分和關(guān)節(jié)炎指數(shù)評分,計(jì)算關(guān)節(jié)腫脹數(shù)。造模后d14(炎癥初期)、d22(炎癥高峰期)處死大鼠,流式細(xì)胞儀檢測炎癥初期和炎癥高峰期脾臟總T細(xì)胞(CD3^+CD4^+)、記憶T細(xì)胞(CD4^+CD44^+)、未致敏T細(xì)胞(CD4^+CD62L^+)的細(xì)胞亞群和Th17(CD4^+I(xiàn)L17^+)細(xì)胞亞群比例。d22取心、肝、脾、肺、腎、踝關(guān)節(jié)、腸系膜淋巴結(jié)組織,光鏡觀察各組織的病理學(xué)改變。結(jié)果 大鼠免疫后10d出現(xiàn)繼發(fā)側(cè)足爪紅腫,炎癥初期(d10~d14)和炎癥高峰期(d19~d22)AA大鼠的全身評分、關(guān)節(jié)炎指數(shù)評分和關(guān)節(jié)腫脹數(shù)有不同程度的增加。踝關(guān)節(jié)病理見滑膜組織增生、炎細(xì)胞浸潤,血管翳出現(xiàn)等,且心、肝、脾、肺、腎、腸系膜淋巴結(jié)組織的病理也有不同程度的炎性變化。與正常組比較,炎癥初期AA大鼠脾臟總T細(xì)胞、記憶T細(xì)胞和未致敏T細(xì)胞亞群比例都無明顯變化、炎癥高峰期AA大鼠脾臟總T細(xì)胞、記憶T細(xì)胞和未致敏T細(xì)胞亞群比例明顯升高;Th17細(xì)胞亞群比例在炎癥初期和高峰期都明顯升高。結(jié)論 AA大鼠的T細(xì)胞亞群在炎癥初期和高峰期出現(xiàn)不同的變化特點(diǎn);在大鼠AA不僅有關(guān)節(jié)局部的病理表現(xiàn),且全身主要臟器組織均有受累。
Abstr:Aim To observe the variation of T-cell subsets in spleen in different courses of disease and tissue histopathological changes in rats with adjuvant arthritis( AA). Methods The model of rat AA was induced by complete Freund 's adjuvant into the right hind metatarsal footpad of 20 male Lewis rats. Arthritis was evaluated by total score,arthritis index and paw swelling number. The percentage of total( CD3^+CD4^+), memory( CD4+CD44+), no-sensitizated( CD4^+CD62L^+) and Th17( CD4^+I(xiàn)L-17^+) on CD4+T cells in spleen were detected by flow cytometry on days 14 and 22 after immuniozation. The histopathological evaluation of heart,liver,spleen,lung,kidney,ankle joints and mesenteric lymph nodes were examined by hematoxylin and eosin( HE) stain. Results The onset of secondary arthritis appeared on about day 10 after immunization,with a peak onset on day 19. Systematic arthritis symptoms included front and hind paw swelling and redness( swelling was more apparent in the front rather than hind paws),nodule formation and redness on the ears,connective tissue swelling and redness of the nose and evident nodules and redness on the tail. Total score,arthritis index and the paw swelling numbers were enhanced in AA rats.Pathohistology of ankles joints showed hyperplasia,pannus formation,and inflammatory cells infiltration in AA rats. And pathohistology of heart,liver,spleen,kidney and mesenteric lymph nodes showed inflammatory changes. Compared with normal group,there was no change in the percentage of total,memory and nosensitizated CD4+T cell in the early stage inflammation but they were significantly enhanced in the peak of inflammation. Th17 cell subsets were significantly enhanced in both of the early and peak of inflammation.Conclusio
