【摘要】：目的探討顳骨手術(shù)中面神經(jīng)垂直段的走行特點(diǎn)及其定位方法。方法應(yīng)用30例(60側(cè))成人尸頭標(biāo)本,(其中3例新鮮標(biāo)本,27例10%福爾馬林固定標(biāo)本)。在手術(shù)顯微鏡下模擬乳突部位手術(shù),暴露面神經(jīng)垂直段及其周?chē)Y(jié)構(gòu),觀察面神經(jīng)垂直段的走行特點(diǎn),與周?chē)Y(jié)構(gòu)的關(guān)系以及面神經(jīng)管周?chē)墓琴|(zhì)變化特點(diǎn)。結(jié)果面神經(jīng)垂直段長(zhǎng)度(16.31±1.29)mm,面神經(jīng)垂直段至后半規(guī)管距離(2.65±0.85)mm,面神經(jīng)垂直段至骨性外耳道口距離(17.28±1.09)mm,面神經(jīng)垂直段至頸靜脈球距離(3.58±1.32)mm,面神經(jīng)垂直段至外耳道后壁距離(4.67±0.39)mm,面神經(jīng)垂直段至乙狀竇距離(6.42±2.65)mm.面神經(jīng)隱窩外側(cè)氣房存在者88.33%,面神經(jīng)隱窩外側(cè)氣房位于面神經(jīng)管外0~2mm.接近面神經(jīng)管部位血運(yùn)豐富,為營(yíng)養(yǎng)面神經(jīng)的微血管所在.錐隆起在面神經(jīng)管稍靠?jī)?nèi)側(cè),附著在面神經(jīng)管內(nèi)1/3以?xún)?nèi)位置,面神經(jīng)垂直段位于二腹肌嵴的前端并稍靠?jī)?nèi)。鼓乳裂消失就接近面神經(jīng)垂直段。結(jié)論面神經(jīng)隱窩外側(cè)氣房、錐隆起、二腹肌嵴、鼓乳裂等,能夠幫助我們定位面神經(jīng)垂直段。解剖學(xué)實(shí)踐、體會(huì)面神經(jīng)垂直段與接近面神經(jīng)過(guò)程中骨質(zhì)的改變、顏色改變,才能更準(zhǔn)確地定位面神經(jīng)。
[Abstract]:Objective to investigate the characteristics and localization of the vertical segment of facial nerve in temporal bone surgery. Methods 30 adult cadaveric head specimens (including 3 fresh specimens, 27 cases with 10% formalin fixation) were used. The vertical segment of the facial nerve and its surrounding structure were exposed under the operating microscope. The characteristics of the vertical segment of the facial nerve and the relationship between the vertical segment of the facial nerve and the surrounding structure as well as the bone changes around the facial nerve canal were observed. Results the length of the vertical segment of the facial nerve was 16.31 鹵1.29 mm, the distance between the vertical segment of the facial nerve and the posterior semicircular canal was 2.65 鹵0.85 mm, the distance between the vertical segment of the facial nerve and the orifice of the osseous external auditory canal was 17.28 鹵1.09 mm, the distance between the vertical segment of the facial nerve and the jugular vein ball was 3.58 鹵1.32 mm, and the distance between the vertical segment of the facial nerve and the posterior wall of the external auditory canal was 4.67 鹵0.39 mm. The distance from vertical segment of facial nerve to sigmoid sinus was 6.42 鹵2.65 mm. The lateral chamber of the facial recess was 88.33 mm, and the lateral chamber of the recess of the facial nerve was 0 ~ 2 mm outside the facial nerve canal. Near the facial canal, the blood flow is abundant, which is the microvessel of the nutritive facial nerve. The conical eminence is located slightly on the medial side of the facial canal and within a third of the facial canal. The vertical segment of the facial nerve is located at the front and slightly inside of the bicentric crest. The cleavage of tympanum is close to the vertical segment of the facial nerve. Conclusion the lateral chamber of the facial recess, conical eminence, bicentric ridge, tympanic and mammary fissure can help us locate the vertical segment of the facial nerve. In order to locate the facial nerve more accurately, the changes of bone and color in the vertical segment of the facial nerve and the approach to the facial nerve can be realized in anatomical practice.
