【摘要】:脊髓損傷(spinal cord injury,SCI)是臨床常見創(chuàng)傷,其致殘率與致死率高,一直是基礎(chǔ)和臨床醫(yī)學(xué)的研究熱點(diǎn)。近年來,為探討脊髓損傷修復(fù)的機(jī)理進(jìn)行了大量的實(shí)驗(yàn)研究取得許多有價(jià)值的實(shí)驗(yàn)結(jié)果,同時(shí),動(dòng)物模型研究也取得了進(jìn)一步的完善,多種類型的脊髓損傷模型廣泛應(yīng)用于基礎(chǔ)研究中,但在不同種類動(dòng)物實(shí)驗(yàn)中評價(jià)脊髓損傷存在很大差異。建立脊髓半橫斷模型,為研究脊髓損傷后神經(jīng)修復(fù)及運(yùn)動(dòng)功能改善的研究提供良好的實(shí)驗(yàn)?zāi)P?才有可能深入探討脊髓損傷修復(fù)及再生機(jī)理,使其基礎(chǔ)研究得到新的突破,為臨床治療提供有價(jià)值的實(shí)驗(yàn)數(shù)據(jù)。作者用靈長類恒河猴作為實(shí)驗(yàn)動(dòng)物,在一定程度上成功模擬了人類脊髓損傷后的病理改變和臨床表現(xiàn),建立標(biāo)準(zhǔn)可重復(fù)的模型。 實(shí)驗(yàn)方法:全麻的恒河猴頸部后正中切口顯露C3-5椎體平面脊髓,于C4用虹膜刀切開脊髓左半側(cè)0.5cm,通過體感誘發(fā)電位(somatosensory evoked potential SEP)監(jiān)測確定波幅降低50%,術(shù)后24h、7d重新測量SEP;觀察肢體運(yùn)動(dòng)情況,進(jìn)行運(yùn)動(dòng)評分、用數(shù)碼相機(jī)錄像分析肢體活動(dòng)構(gòu)成。 結(jié)果表明:術(shù)后24h、7d作為觀察時(shí)間窗,肢體運(yùn)動(dòng)評分由術(shù)前的6分降到1分。SEP顯示前肢正中神經(jīng)術(shù)前潛伏期為15.54±2.69 ms,術(shù)后24h為22.36±4.19ms、7d為23.14±3.89ms,延長明顯(P0.01)。術(shù)前波幅為13.04±4.95μv,術(shù)后24h為5.28±3.02μV、7d為5.32±2.75μV,降低明顯(P0.01)。后肢脛后神經(jīng)術(shù)前潛伏期22.84±3.59 ms;術(shù)后24h為33.02
[Abstract]:Spinal cord injury (sci) is a common clinical trauma with high disability rate and fatality rate. It has always been a hot topic in basic and clinical medicine.In recent years, in order to explore the mechanism of spinal cord injury repair, a large number of experimental studies have been carried out to obtain many valuable experimental results, at the same time, the animal model research has been further improved.Many types of spinal cord injury models are widely used in basic research, but there are great differences in the evaluation of spinal cord injury in different kinds of animal experiments.To provide valuable experimental data for clinical treatment.Using primate rhesus monkey as experimental animal, the pathological changes and clinical manifestations of human spinal cord injury were successfully simulated to a certain extent, and a standard repeatable model was established.Methods: the spinal cord of C3-5 vertebra was exposed through the median incision of the neck of rhesus monkey under general anesthesia.The left half of spinal cord was cut with iris knife at 0.5 cm. Somatosensory evoked potential (SEP) was monitored to determine the decrease of amplitude by 50%, and then to re-measure SEP 24 hours after operation, to observe the movement of limbs, to score the motion, and to analyze the composition of limb motion by digital camera video recording.The amplitude of the wave was 13.04 鹵4.95 渭 v before operation and 5.28 鹵3.02 渭 V at 24 hours after operation. The amplitude was 5.32 鹵2.75 渭 V on the 7th day after operation.The latency of posterior tibial nerve in hind limb was 22.84 鹵3.59 Ms before operation and 33.02 in 24 hours after operation.
